Cuerpo extraño bronquial en paciente con fijación intermaxilar y mandibular: presentación de un caso / Bronchial foreign body with inter-maxillary and mandibular fixations: presentation of case

RESUMEN

La introducción fortuita de un cuerpo extraño en las vías respiratorias es uno de los accidentes más dramáticos que pueden acontecer y motivo de reflexión sobre la tenue frontera que separa la vida de la muerte. Se presenta un caso que fue intervenido quirúrgicamente por presentar Le Fort III con Fractura mandibular y dentoalveolar al que se le realizó fijación esquelética interna y fijación intermaxilar elástica, al mes acude a su médico de asistencia con manifestaciones respiratorias (tos, fiebre, expectoración y ligera dificultad respiratoria) quien indica Radiografía de tórax constatándose la presencia de un cuerpo extraño en Bronquio Derecho (muela), anunciándose de urgencia por el servicio de ORL para la extracción del cuerpo extraño. Dada la alta incidencia de complicaciones asociadas a este tipo de lesión, el reto que constituye para el anestesiólogo permeabilizar la vía aérea en este caso que presenta fijación de mandíbula la que se retira para intentar el proceder y la posibilidad de desestabilizarse con la maniobra de la laringoscopia e intentar mantener una adecuada ventilación, se presenta el caso y se realiza una revisión bibliográfica sobre la conducta anestésica durante este proceder.

ABSTRACT

The accidental ingestion of a foreign body in the respiratory airways is one of the more dramatic accidents that may occur, which are a reason for reflection on the brink of life and death. The case of a patient who underwent surgery and displayed Le Fort III fracture with mandibular and dento-alveolar fractures, internal skeletal fixation and elastic inter-maxillary fixation were performed. At one mo the patient goes to his doctor´s with respiratory manifestations (cough, fever, expectoration and slight respiratory difficulty). The thoracic x-ray shows the presence of a foreign body in the right bronchus (tooth) announcing it as urgent by the service of Otolaryngology for its extraction. Given the high incidence of complications associated to this type of injury, It constitutes a challenge for the anesthesiologist to make the airway permeable; in this case presenting fixation of mandible, which is taken away to go ahead with the procedure and the possibility of destabilizing it with a laryngoscopy maneuver and trying to keep a adequate ventilation. The case is presented and a bibliography is reviewed on the anesthetic behavior during this procedure.
