ABSTRACT
On first appearance a number of conditions may look remarkably similar to a pneumoperitoneum, and these must be considered in every doubtful case. If they are not recognized it may lead to an unnecessary laparotomy in search of a perforated loop. Chilaiditis syndrome interposition of bowel between the liver and diaphragm on the right may exactly simulate a pneumoperitoneum and is usually discovered by chance during the study of another event. The case presented involves a 13 year old girl mentally ill with Chilaiditi s syndrome who was successfully managed with conservative therapy. At follow up, the patient reported marked improvement of abdominal symptoms.
