Objetivo:
Avaliar o índice de malignidade através do valor preditivo positivo das lesões mamográficasclassificadas nas diferentes subclasses do BI-RADS® 4 em pacientes submetidas a biópsia histológica.Materiais e Métodos:
Foram analisadas 102 mamografias provindas de serviços de Radiologia da região de Itajaí/SC, independentemente de sua indicação(Screeaning e/ou achado de exame físico), onde os laudos apresentavam a classificação BI-RADSÕ 4 estratificada conforme proposta pelo American College of Radiology em 2003. Para o cálculo do valor preditivo positivo correlacionou-se os dados histológicos de fragmentos colhidos por Core-Biopsy® ou exérese cirúrgica. Resultados submetidos a teste qui-quadrado considerando p<0,05.Resultados:
Os autores encontraram 62 casos de neoplasia maligna com VPP para as categorias A, B e C de 40,7 %, 47,2 % e 87,2 % respectivamente (quiquadrado= 18,72 / p=0,0001).Conclusão:
Os autores concluíram haver uma correlação direta entre o BI-RADSÕ 4 e o risco de malignidade, encontrando elevação progressiva do VPP nas subclasses A, B e C.ABSTRACT
Objective:
To evaluate the risk of malignancy index using the positive predictive value (PPV) of mammographic lesions, classified according to the subcategories of the Breast Imaging-Reporting and DataSystem (BI-RADS) category 4, in patients who underwent histological biopsy. Materials andMethods:
One hundred and two mammograms, collected from radiology facilities locatedin the region of Itajaí, SC, Brazil, were analyzed regardless of their original indication (screening mammogram and/or findings of physical examination).According to the medical records, all mammograms were classified as BI-RADS category 4, based on theAmerican College of Radiology (ACR) BI-RADS categories (2003). For the calculation of PPV, histopathological data of Core Biopsy or surgicallyexcised specimens were correlated. Chi-square analysis of the results was performed at a significance level of0.05 (p<0.05).Results:
We found 62 cases of malignant neoplasia with PPV of 40.7%, 47.2% and 87.2% for the subcategories 4A, 4B and 4C, respectively (chi-square = 18.72; p=0.0001)Conclusion:
We conclude that there is a direct correlation between BI-RADS category 4 and risk ofmalignancy, with a progressive increase in PPV for the subcategories 4A, 4B and 4C.
