A efetividade de um protocolo fisioterapêutico de terapia manual para o alívio da dor de pacientes com cefaléia tensional e alterações da coluna cervical / The effectiveness of manual therapy for the pain relief in patients with tensional chronic headache and cervical column alterations

RESUMO
Objetivo: Verificar a efetividade de um protocolo fisioterapêutico de terapia manual na intensidade dolorosa e no limiar de dor por pressão (LDP) em indivíduos com cefaléia tensional e alterações posturais e degenerativas da coluna cervical. Método: Foram avaliados 24 sujeitos (22 mulheres e 2 homens) que responderam a questões referentes à intensidade, duração e interferência da dor nas AVDs. Posteriormente, foram submetidos a radiografias nas posições AP e perfil. Por meio das radiografias foram medidas as angulações da lordose cervical, classificando os indivíduos em portadores (IP) de alteração cervical. Em seguida foram escolhidos 3 IP para realização da segunda etapa que consistiu no tratamento com base na terapia manual (alongamento, massagem e mobilização vertebral), por 10 sessões. Foi usada a linha de base múltipla do tipo ABA e as variáveis controladas foram intensidade dolorosa (EVA) e LDP (algômetro de pressão). Resultado: O tratamento teve efetividade no alívio da cefaléia e no aumento do LDP. O protocolo mostrou-se efetivo quando analisadas as variáveis antes e após cada sessão de tratamento. Conclusão: Os dados sugerem que o protocolo foi efetivo no alívio da cefaléia e no aumento do LDP dos indivíduos com cefaléia tensional portadores de alterações da postura cervical.
ABSTRACT
Objective: To verify the effectiveness of a physiotherapeutic protocol of manual therapy in the painful intensity and in the pain threshold for pressure (LDP) in individuals with tensional migraine and postural and degenerative alterations of the cervical column. Method: 24 individuals were evaluated (22 women and 2 men) that answered questions referring to the intensity of the pain, duration and the interference of the pain in the daily life activities (AVDs). Later, they were submitted to X rays in the positions AP and pro- file. Through the X rays, the angulations of the cervical lordosis were measured classifying the individuals in bearers (IP) of cervical alterations. After that 3 IP were chosen for the accomplishment of the second stage that consisted of a treatment based in the manual therapy (prolongation, massage and vertebral mobilization) for 10 sessions. The line of multiple base of the type ABA was used and the controlled variables were painful intensity (EVA) and LDP (pressure algometer). Results: The treatment had effectiveness in the relief of the migraine and in the increase of LDP. Th e protocol was effective when the variables were analyzed before and after each session of treatment. Conclusion: The data suggest that the protocol was effective in the relief of the tension and in the increase of LDP of the individuals with tension headache, bearers of alterations in the cervical posture.
