Correlación entre bipolaridad, intento suicida y rasgo impulsividad en un grupo de pacientes en tratamiento ambulatorio por trastorno del ánimo / Correlation betwen bipolarity, suicide attempt and impulsivity trait in a group of outpatients in treatment with affective disorders

ABSTRACT

Objective: Not only in clinical practice but also in diverse studies and revisions, there is a clear correlation between suicide and impulsivity. In affective disorders, this association seems to be particularly notorious in patients with bipolar disorder. The objective of this investigation was to evaluate if the impulsivity trait is associated to the suicidal attempt in bipolar patients, independent to mood compromise. Methods: The application of the Barratt Impulsivity Rating Scale (BIS-11) to a total of 41 outpatients. The distribution was 11 bipolar patients with attempt, 19 bipolar patients without attempt and 11 non bipolar patients with attempt. Limitations: The non bipolar group included patients with episodes not remitted; all the patients were medicated; there was no healthy control group; small size of the sample. Discussion: In our study the association between the impulsivity trait and suicidal attempt was confirmed in bipolar patients; the higher score in the cognitive subscale in the non bipolar group could be oriented to consider the impulsivity trait as a risk factor of suicide attempts, independent of the type of affective disorder. These findings can have preventive and therapeutic implications.

RESUMEN

Objetivo: Tanto en la práctica clínica como en diversos estudios y revisiones se evidencia la correlación entre suicidio e impulsividad. Entre los trastornos del ánimo esta asociación parece particularmente notoria en los pacientes con trastorno bipolar. El objetivo del presente trabajo fue evaluar si en el trastorno bipolar el rasgo impulsividad puede o no asociarse como factor de riesgo de intento suicida en forma independiente al compromiso del ánimo. Métodos: Se aplicó la Escala de Impulsividad de Barratt (BIS-11) a un total de 41 pacientes ambulatorios distribuidos en 11 pacientes bipolares con intento, 19 pacientes bipolares sin intento y 11 pacientes no bipolares con intento. Resultados: Se observó un puntaje total significativamente mayor en pacientes bipolares con intento que en los no intentadores, y como hallazgo en bipolares II respecto de bipolares I. Fue también significativamente mayor el puntaje en la subescala cognitiva en pacientes intentadores no bipolares que en bipolares sin intento. Limitaciones El grupo no bipolar incluyó cuadros no remitidos ; todos los pacientes estaban medicados; no hubo controles sanos; tamaño pequeño de la muestra Discusión: En nuestro estudio se confirma la asociación entre rasgo impulsividad e intento suicida en pacientes bipolares; el mayor puntaje en la subescala cognitiva en intentadores no bipolares pudiera orientar a considerar el rasgo impulsividad como un factor de riesgo de intento suicida independiente del tipo de trastorno del ánimo. Estos hallazgos pueden tener implicancias preventivas y terapéuticas.
