É possível aferir a qualidade da representação dos usuários nos conselhos de saúde? / Is it possible to assess the quality of user representation in local health care councils?

RESUMO

Este artigo é produto da pesquisa 'A qualidade da representação dos usuários nos Conselhos Distritais de saúde do Rio de Janeiro e a dimensão associativa', realizada em 2002, sob a coordenação da autora e financiada pelo Programa Pesquisa estratégica da ENSP/FIOCRUZ. O trabalho examina a possibilidade de aferir a qualidade da representação dos conselheiros do segmento dos usuários nos Conselhos de Saúde. A análise baseia-se em resultados de pesquisa realizada junto aos Conselhos Distritais de Saúde do Município do Rio de Janeiro, a qual mostrou serem as associações demoradores as que têm o maior número de conselheiros. A exposição esquadrinha a lógica de funcionamento dessas associações e as relações entre elas e seus representantes. As conclusões apontam entre outros aspectos, para a 'oligarquização' dessas entidades e a 'elitização1 dos conselheiros por elas indicados. Essestraços, aliados ao insulamento das associações nas deliberações internas e á escassa participação da comunidade, impediriam definir parâmetros para avaliar o desempenho desses conselheiros, em particular no que diz respeito ao controle social. Sugere-se, por último, incluir a dimensão associativa dos conselhos em iniciativas de capacitação e pesquisa.

ABSTRACT

This article is a result of the research study entitled 'The quality of user representation in the District Health Councils of Rio de Janeiro and the associative dimension', conducted in 2002 under the coordination of the author herself, and funded by the ENSP/FIOCRUZ Strategic Research program. The article attempts to determine whether it is possible to evaluete the quality of user representation in the public health care councils. The arguments are based on the results of research concerning the District Health Councils of Rio de Janeiro, Which showed that the neighborhood association have the greatest number of counselors among those representing users of public health services. The analysis focuses on the functioning of those associations, as well as on their relationships with the counselors they appoint and with the community. Amongst other aspects, the conclusions point towards the 'oligarquization' of these associations and the 'elitization' of counselors. These characteristics, allied to insulated decision-making, and low community participation, serve to hinder the definition of the parameters by which to evaluate the performance of the counselors, especially with respect to social control. Finallly, it is suggested that research and training initiatives adopt an associative dimension.
