Objetivo:
Analisar os erros do processo diagnóstico, identificação da forma clínica e tratamento dos pacientes com neurossífilis.Método:
Foram estudados 10 pacientes com neurossífilis e observados os diagnósticos de alta, incluindo a forma clínica, intaerpretação dos exames laboratoriais solicitados e condutas terapêuticas utilizadas. Os erros foram classificados em ômaiorõ, ômenorõ e ôincoerênciaõ.Resultados:
Não houve associação entre diagnóstico e tratamento corretos (χ 2 Yates=0,03; g.l.=1; p = 0,8630) e o coeficiente de concordância foi péssimo (k = 0,19; z=1,04; p=0,1503). A associação entre forma clínica e tratamento também mostrou concordância péssima (k=0,19; z=1,04; p=0,8630), e baixa acurácia (sensibilidade = 28,6%; especificidade = 100,0%; valor preditivo positivo = 100,0%; valor preditivo negativo = 37,5%). Os intervalos de confiança para os erros foram erro maior de diagnóstico (8,3% a 62,3%); erro maior de tratamento (29,1% a 85,8%); incoerência de tratamento (3,5% a 52,0%); erro menor de forma clínica (8,3% a 62,3%).Conclusão:
Os erros mais comuns foram a incerteza diagnóstica, a falta de definição da forma clínica e o tempo de tratamento.ABSTRACT
Objective:
To analyze the diagnostic errors and to identify the clinic form and treatment in patients with neurosyphilis.Method:
Ten patients with neurosyphilis were studied. Discharge diagnosis was observed, including the clinic form, interpretation of the laboratory results and the treatment. The errors were classified in ômajorõ, ôminorõ and ôincoherenceõ.Results:
There was not association between correct diagnosis and treatment (χ 2Yates=0.03; g.l.=1; p = 0.8630), and the concordance coefficient was very low (k = 0.19; z=1.04; p=0.1503). Association between clinical form and treatment also showed vary low concordance (k=0.19; z=1.04; p=0.8630) and low accuracy (sensitivity=28.6%; specificity=100.0%; positive predictive value=100.0%; negative predictive value=37.5%). The confidence intervals for errors were major error of diagnosis (8.3% a 62.3%); major error of treatment (29.1% to 85.8%); incoherence of treatment (3.5% to 52.0%); minor error of clinical form (8.3% to 62.3%).Conclusion:
The most frequent errors were diagnostic uncertainty, indefinite clinical form and time of treatment.
