Facoemulsificación y queratotomía astigmática / Phacoemulsification and astigmatic keratotomy

RESUMEN

El control del astigmatismo posoperatorio es uno de los retos mayores del cirujano de catarata, por lo que se ha visto obligado no solo a perfeccionar su técnica quirúrgica, sino además, a buscar herramientas que complementen este trabajo, y así se utilizan diferentes opciones que incluyen incisiones en córnea clara en el meridiano más plano, queratotomía astigmática, implantes de lentes intraoculares tóricos, incisiones opuestas en córnea clara y incisiones relajantes limbales o incisiones relajantes corneales periféricas. Nos trazamos como objetivo determinar los resultados obtenidos con la queratotomía astigmática por incisiones relajantes limbales en la cirugía de catarata por facoemulsificación con la técnica de pre chop, evaluar el componente esférico esperado y el obtenido, determinar el cilindro queratométrico preoperatorio y el posoperatorio, los resultados de la mejor agudeza visual corregida preoperatorio y posoperatoria. Se realizó un estudio descriptivo, prospectivo de corte trasversal, cuyo universo estuvo constituido por 198 pacientes (ojos) con diagnóstico de catarata presenil y senil que recibieron tratamiento quirúrgico con la técnica pre chop por facoemulsificación más queratotomía astigmática por incisiones relajantes limbales, en el Centro de Microcirugía Ocular en el período comprendido desde enero de 2002 hasta enero de 2005. El seguimiento de los pacientes fue de 3 meses. Se obtuvo como resultado un mejoramiento del componente esférico, y un cilindro queratométrico que aumentó discretamente. En todos los casos se logró una mejoría visual en líneas de visión y un tiempo de ultrasonido medio bajo.

ABSTRACT

The control of postoperative astigmatism is one of the biggest challenges met by the cataract surgeon, who has been forced for these reason not only to improve the surgical technique to be performed but also to look for tools that supplement this job. In this way, different choices are used including clear corneal incisions in the steep meridian, astigmatic keratotomy, toric intraocular lens implantation, opposite clear corneal incisions and limbal relaxing incisions or peripheral relaxing corneal incisions. Our objective was to determine the results of astigmatic keratotomy with limbal relaxing incisions in cataract surgery based on phacoemulsification using Pre chop technique; to evaluate the expected and the achieved spheral component; to determine the pre-operative and post-operative keratometric cylinder; and analyze the results of the best corrected visual acuity before and after surgery. A prospective, descriptive and cross-sectional study was conducted in 198 patients (eyes) diagnosed with pre-senile and senile cataract and surgically treated with Pre chop technique-based phacoemulsification plus astigmatic keratotomy with limbal relaxing incisions at the Center of Ocular Microsurgey in the period ranging from January 2002 to January 2005. The patients were followed up for 3 months. As a result, spheral component improved, and the keratometric cylinder slightly increased. In all the cases, the visual lines got better and average ultrasound time was low.
