Percepción de la calidad de vida por enfermos sometidos a tratamientos de hemodiálisis o trasplante renal: estudio comparativo / Perception of the quality of life of patients undergoing haemodialysis or kidney transplant: a comparative study

RESUMEN

Se sabe que los pacientes afectos de enfermedad renal crónica terminal sometidos a hemodiálisis (HD) o trasplante renal (TR) tienen un deterioro importante de su calidad de vida (CV), no obstante, el implante de un nuevo riñón normalmente funcionante se acompaña de una mejor percepción de la CV y, por ende, un mayor ajuste al tratamiento. Se realizó un estudio para comparar la percepción de la CV en 2 grupos de pacientes: uno bajo tratamiento de HD (GI = 20 enfermos) y otro de TR (G2 =32 pacientes) que cumplieran los criterios de inclusión establecidos, para corroborar la anterior aseveración. Se les aplicó una encuesta con parámetros generales, clínicos y psicológicos. El equipo de atención a estos enfermos hizo una valoración cuantitativa de la misma. Se halló que: los pacientes con TR tenían buena percepción de su CV, superior al compararla con la de los enfermos tratados con HD; las mujeres, los jóvenes y el nivel de escolaridad medio constituyeron los grupos de enfermos con mejor aceptación de su enfermedad y del tratamiento aplicado, mientras que el tiempo prolongado de HD, los niveles bajos de albúmina, hemoglobina y la alta tasa de hospitalización se correlacionaron negativamente con la percepción de su CV, las mayores preocupaciones de estos enfermos se centraban en el apoyo y las relaciones familiares, le siguieron en importancia, la esfera sexual y la reincorporación laboral.

ABSTRACT

It is known that those patients suffering from end-stage chronic renal disease that undergo haemodialysis (HD) or renal transplant (RT) experience a significant deterioration of their quality of life; however, the implant of a new kidney that works normally leads to a better perception of the quality of life and, therefore, to a better adjustment to the treatment. A study was conducted to compare the perception of the quality of life in 2 groups of patients: one under haemodialysis treatment (G1 = 20 patients), and the other with renal transplant (G2 = 32 patients) that fulfilled the criteria of inclusion established to corroborate the above affirmation. A survey with general, clinical and psychological parameters was done. Their medical team made a quantitative evaluation of it, and it was found that the patients with RT had a good perception of their QL that was higher on comparing it with that of the patients treated with HD. The females, the young patients and those with middle educational level were the groups showing a better acceptance of their disease and of the treatment applied; whereas the prolonged time of treatment, the low levels of albumin, haemoglobin, and the high rate of hospitalization were negatively correlated with their perception of the quality of life. The greatest concerns of these patients focussed on the family support and relations, followed by the sexual sphere and their reincorporation to work.
