O laudo mamográfico e a recomendação de conduta / The mammographic report and the recommendation of the course of action

RESUMO
A padronização dos laudos mamográficos, inicialmente realizada através do BI-RadsTM e adotada com inserção de outros parâmetros (inclusive recomendação de conduta dada pelo radiologista) pelo Colégio Brasileiro de Radiologia, pela Sociedade Brasileira de Mastologia e pela Federação Brasileira das Sociedades de Ginecologia e Obstétrícia (Febrasgo) tem grandes méritos, mas comporta reflexões pontuais para otimizar e racionalizar a sua utilização. Em especial, devem-se aprofundar as reflexões sobre a recomendação de conduta em casos de classificação III e em alguns casos de classificação IV. Este artigo discorre sobre os pontos desta padronização que merecem ser avaliados mais profundamente. São também feitas considerações sobre a periodicidade e a população-alvo do rastreamento mamográfico.
ABSTRACT
The standardization of mammographic reporting, at first through the BI-RadsTM lexicon, and subsequently adopted with modifications (including a recommendation of the course of action) by the Brazilian Societies of Radiology, Mastology and Gynecology and Obstetrics has many qualities. Nevertheless, there is a need for further reflexions in order to racionalize and optimize its use. The recommendation of treatment deserves special attention, specially procedures or short term follow-up in cases of BI-RadsTM III, and in some cases of BI-RadsTM 0 and IV. This paper makes more detailed analyses about selected issues of the recommendations of the Brazilian societies. Also, comments are made on the screening intervals and the age group to be included in the screening.
