Curva nacional de distribución de peso al nacer según edad gestacional en el Perú / National curves of birth weight distribution according to gestational age in Perú

RESUMEN

El peso de nacimiento en relación con la edad gestacional, tiene valor pronóstico y sirve para el manejo clínico del recién nacido. La OMS recomienda patrones de crecimiento fetal en los centros perinatológicos, pues se detectan diferencias según países y condiciones de vida. Objetivo: Obtener curvas de crecimiento intrauterino nacional de acuerdo a criterios propuestos por la OMS. Material y métodos: Enrolamiento prospectivo de recién nacidos vivos sucesivos en 29 Hospitales del Ministerio de Salud, nacidos en el año 2005, se utilizó técnicas antropométricas recomendadas por el CLAP-OPS/OMS, diagnóstico de edad gestacional por FUM; se seleccionaron recién nacidos sin factores de riesgo de retardo de crecimiento intrauterino; se usó Sistema Informático Perinatal (SIP2000), Visual Fox, Microsoft Excel y SPSS. Se obtuvo percentiles, medias, desviación estándar y test estadísticos necesarios. Resultados: 50 568 RN vivos fueron seleccionados de 99 439 nacimientos, su peso promedio de nacimiento fue 3011-3506 g. y el percentil 10 de peso fue 2435-3030 g. a las 37 y 42 semanas respectivamente. Se consideró además de los percentiles 10, 50 y 90, los percentiles 2,5 y 5 para clasificar al RN PEG en leve, moderado o severo, según se encuentre el peso bajo el percentil 10, 5 y 2,5 respectivamente. Las semanas 39 y 40 de gestación fueron las más representativas (55,8 por ciento), con un promedio de peso de 3295 g. ± 407 g. y 3400 ± 421 g. respectivamente, los percentiles 10, 50 y 90 fueron 2750, 3275, 3810 g. y 2875, 3385, 3930 g. respectivamente. La talla promedio de nacimiento fue 49,7 cm ± 2,3 y 50,1 cm ± 2,3; y el perímetro cefálico 343 mm ± 16 y 345 mm ± 16 a las 39 y 40 semanas respectivamente. Conclusiones: Se presentan familia de curvas de crecimiento intrauterino nacional según peso, talla y perímetro cefálico por edad gestacional en un grupo seleccionado de recién nacidos peruanos.

ABSTRACT

The weight from birth in connection with the gestational age, has prognostic value and it is good for the clinical handling of the newly born one. OMS recommends standards of fetal increment in the centers perinatologicos, because differences are detected according to countries and conditions of life. Objective: To obtain intra-uterine national growth curves according to approaches offered by OMS. Material and method: Prospective enrolamiento of newly born alive successive in 29 Hospitals of the Ministry of Health, born in the year 2005, technical antropométricas was used recommended by CLAP-OPS / OMS, gestational age diagnostic for FUM; they were selected newly born without factors of risk of delay of intra-uterine increment; Data Perinatal (SIP2000), Visual System Fox was used, Microsoft Excel and SPSS. It was considered besides the percentiles 10, 50 and 90, those 'percentiles 2,5 and 5 to classify RN PEG in light, moderate or severe, as is the low weight the percentile 10, 5 and 2,5 respectively. Conclusions: They show up intra-uterine national family of growth curves according to weight, it carves and cephalic perimeter for gestational age in a selected group of newly born Peruvians.
