Papel de la terapia cognitiva en el manejo integral de los trastornos bipolares / Cognitive therapy role in integral management of bipolar disoders

ABSTRACT

In the last decade the therapeutical arsenal for the pharmacological treatment of the bipolar disorder has had an important growth, despite the latter many patients do not reach good levels of global functioning. Some of these patients do not accept or understand the benefits of the use of medications, other stop taking the medication or modify the doses so as to experiment side effects or idiosyncratic reactions and in other opportunities is the relatives who negatively interfere on the treatment; all the above causes a poor treatment adherence and thus a therapeutic failure. Some psychosocial interventions associated to medication have recently shown evidences of its efficacy. The present article presents a review of the diverse psychosocial approaches in the treatment of the bipolar disorder, emphasizing the cognitive therapy.

RESUMEN

En la última década, el arsenal terapéutico para el tratamiento farmacológico del trastorno bipolar ha tenido un importante crecimiento, pese a lo cual muchos pacientes no alcanzan buenos niveles de funcionamiento global. Algunos de éstos no aceptan o no comprenden los beneficios del tratamiento farmacológico de mantención, otros suspenden o modifican la posología al experimentar efectos secundarios o reacciones idiosincrásicas; y en otras oportunidades son los familiares del paciente quienes intervienen negativamente sobre el mismo; todo lo anterior suele llevar a una pobre adherencia y por ende al fracaso terapérutico. Algunas intervenciones psicosociales, asociadas a la medicación, han mostrado recientemente evidencias de su eficacia. El presente artículo presenta una revisión de los diversos enfoques psicosociales en el tratamiento del trastorno bipolar, enfatizando en la terapia cognitiva.
