Rotura del ligamento colateral cubital de la articulación metacarpofalángica del pulgar: reparación anatómica aislada o asociada a un procedimento activo de estabilización? / Rupture of the ulnar collateral ligament of the thumb’s metacarpophalangeal joint: anatomical preparation associated or not to a stabilization active procedure

RESUMEN

La rotura del ligamento colateral cubital de la articulación metacarpofalángica del pulgar constituye la más frecuente de las lesiones ligamentarias de la mano. En este estudio retrospectivo, comparativo y cualitativo se comparan dos modalidades terapéuticas quirúrgicas para el tratamiento agudo de dichas lesiones. Tomando una muestra de 4 pacientes, con esta lesión, de los dividió en dos grupos; grupo1 en cual se realizó la reparación anatómica del ligamento y grupo 2 en el cuál se complementó la reparación anatómica con la transferencia músculotendinosa del extensor propio del dedo índice a la base de la primera falange mediante la técnica de Kaplan. Como criterio de evaluación se tomó en cuenta la estabilidad pasiva y activa, el rango articular, el dolor y el desempeño laboral. Se concluyó que la transferencia músulotendinosa realizada es un procedimiento activo que potencia la reparación y tiene sus principales indicaciones en la población analizada.

ABSTRACT

The most common ligamentous injury of the hand, bis the rupture of the ulnar collateral ligament of the thumb´s metacarpophalangeal joint. This retrospective, comparative and cualitative study, consists of the comparison of two surgical options for its acute treatment. We report 4 cases with this injury that were divided into two groups; group 1 was treated by an anatomic repair while in group 2 the anatomic repair was asociated to a transfer of the extensor indicis proprius tendon to the base of the proximal phalanx using Kaplan´s technique. We evaluated results considering active and pasive stability, range of motion, pain and the recovery of working skills. We conclude that the tendon transfer used, is an active procedure that improves the anatomic repair´s results and has its indications in the group of patients analized.
