Overdenture retida por barra: uma alternativa à prótese total / Bar-reitained overdenture: a complete denture alternative

RESUMO

A perda dentária resulta em diversos prejuízos funcionais, tais como a reabsorção óssea, diminuição da capacidade proprioceptiva, do controle da freqüência e do ciclo mastigatório, e da própria capacidade mastigatória. Nos pacientes candidatos à prótese total esses efeitos são ainda mais danosos e podem comprometer o sucesso desta modalidade de tratamento. Uma maneira de evitar esses efeitos é a manutenção das raízes dentárias remanescentes, e sua utilização como suporte para uma overdenture. Esta opção de tratamento é uma alternativa que, quando bem indicada e realizada, apresenta uma série de vantagens sobre as próteses totais convencionais, como a diminuição da reabsorção óssea, manutenção do ligamento periodontal, da propriocepção, e possibilidade de se utilizar sistemas retentivos para a prótese, melhorando assim a retenção e a estabilidade das mesmas, gerando, conseqüentemente, uma maior satisfação ao paciente. Este artigo relata um caso clínico no qual dois caninos inferiores foram utilizados como suportes para uma overdenture retida pelo sistema barra-clip, apresentando a técnica de confecção e confirmando as overdentures como uma alternativa reabilitadora viável

ABSTRACT

The dental loss results in several functional damages, such as bone resorption, decrease of the proprioceptive capacity and of the control and the capacity of the chewing capacity. In those patients who wear complete dentures those effects are still more harmful and they can commit the success of this treatment modality. Away to avoid those effects would be the maintenance of the remaining dental roots, and their use as support for an overdenture. This treatment modality is an alternative that, when well indicated and accomplished, presents a series of advantages over the conventional complete dentures, as the decrease of the bone resorption, maintenance of the periodontal ligament, of the proprioception, and possibility of using retentive systems for the prosthesis, improving the retention and the stability of them, giving, consequently, a larger satisfaction to the patient. We will present a clinical case in which two inferior canine teeth were used as supports for an overdenture retained by a bar, presenting the technique and ratifying it as a viable alternative restorative treatment
