Atuação fisioterápica após fenolização em paciente com paralisia cerebral do tipo diparética espástica / Physical therapeutic approach after phenol application to a pacient with spastic diparetic cerebral palsy

RESUMO
A paralisia cerebral é uma conseqüência anatomopatológica ou uma anomalia estacionaria que age no SNC em pleno amadurecimento funcional e estrutural, ocorrendo na vida fetal ou nos primeiros meses de vida. O dano cerebral causado pela paralisia cerebral leva à inabilidade, à dificuldade ou ao descontrole de músculos e de certos movimentos do corpo, inclusive a espasticidade. Um recurso de tratamento muito utilizado para diminuir os danos causados pela espasticidade e o bloqueio nervoso periférico por meio do Fenol a 5%, ou da toxina botulínica tipo A. O neurobloqueador age como anestésico nas fibras gama, interrompendo a condução nervosa e o arco reflexo, diminuindo o tônus muscular. O objetivo deste trabalho e avaliar a importância do uso de substâncias neurobloqueadoras periféricas, como o Fenol, para o tratamento fisioterápico, a partir do acompanhamento do tratamento fisioterápico após aplicação do fenol no ramo anterior do nervo obturador direito e esquerdo em uma criança com paralisia cerebral do tipo diparética espástica de membros inferiores. Também foi estudada a comparação gomiométrica das articulações do quadril, joelho e tornozelo bilateralmente pré e pós-fenolização bem como a comparação das habilidades funcionais dos membros inferiores pré e pos-fenolização.
ABSTRACT
Cerebral palsy is an anatomopatologic consequence or a stationary anomaly that acts on the central nervous system during functional and structural development, and can occur during fetal life or in the first months of life. The cerebral damage caused by the cerebral palsy leads fear inability, difficulty or uncontrol of the muscles or of certain movements of body, including spasticity. A kind of treatment very used for reducing the damages caused by spasticity is the peripheral nervous blockade with Phenol 5%, or botulinic toxin type A. The peripheral nervous blockade acts as anesthetic in gama fibers, interrupting the nervous conduction and the reflected arc, diminishing niuscles tones. The objective of this paper was to evaluate the importance of using peripheral nervous blockade substances, as Phenol, for physical therapy, by following up follow the physical therapeutic treatment after Phenol application in the previous branch of the right and left obturator in a child with spastic diparetic cerebral palsy affecting the lower limbs. Goniometric measures of bilateral hip, knee and ankle joints as well as functional abilities were compared before and after phenol application.
