O impacto do declínio cognitivo, da capacidade funcional e da morbidade de idosos com doença de Alzheimer na sobrecarga dos cuidadores / The impact of cognitive, functional and mobility decline of elderly with Alzheimer disease on their cargivers' burden

RESUMO

O objetivo foi avaliar capacidade funcional, mobilidade e função cognitiva de idosos com a doença de Alzheimer (DA), bem como o nível de sobrecarga de seus cuidadores, e investigar a associação entre essas variáveis. Foram selecionados 28 idosos (77,82 - 8,28 anos) diagnosticados com DA por meio do manual diagnóstico e estatístico das doenças mentais e da Classificação Internacional de Doenças; também os respectivos cuidadores (57,96 - 13,88 anos), todos participantes da Associação Brasileira de Alzheimer em Goiás. Foram avaliadas função cognitiva, mobilidade e capacidade funcional dos idosos por meio do Mini-exame do estado mental, Southampton Assessment of Mobility e Disability Assessment for Dementia, respectivamente. O nível de sobrecarga dos cuidadores foi avaliado por meio da Zarit Burden Interview. As correlações foram calculadas pelo teste de correlação de Spearman e o nível de significância, fixado em 0,05. Obtiveram-se correlações significativas entre o nível cognitivo e a escolaridade dos idosos (r=0,389; p=0,041), nível de funcionalidade dos idosos e nível de sobrecarga dos cuidadores (r=-0,398; p=0,036), e mobilidade e tempo de diagnóstico da DA (r=0,401; p=0,042). Os resultados sugerem que o deficit cognitivo não interferiu na capacidade funcional e a perda cognitiva foi proporcionalmente maior que a perda da mobilidade. Como essas mudanças desencadeiam maior dependência, observou-se também um aumento da sobrecarga do cuidador...

ABSTRACT

To evaluate the functional capacity, mobility and cognitive function of Alzheimer's Disease (AD) elderly and the level of caregivers burden, and to investigate association between these variables. It were selected 28 elderly (77,82 - 8,28 years old) diagnosed with AD by the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) and the Internacional Statistical Classification of Diseases and Related Health Problems (ICD-10) and their caregivers (57, 96 - 13,88 years old), all of them participants of Brazilian Associantion of Alzheimer/Goiás. It was assessed cognitive function, mobility and functional capacity by the Mini-Mental State Examination, Southampton Assessment of Mobility and Disability Assessment for Dementia, respectively. The caregivers burden was assessed by the Zarit Burden Interview. The correlations were made by Spearman correlation test and the level of significance of 0,05 was considered. It was found significant correlation between the cognitive level and educational level (r=0,389, p=0,041), functional capacity level of elders and caregivers burden (r=-0,398 p=0,036), mobility and time of Alzheimer's diagnosis (r=0,401, p=0,042). Data of this study suggested that the cognitive decline didn’t interfere in the incapacity. The cognitive loss was proportionally greater than the loss of mobility.
