RESUMEN
Los tumores estromales gastrointestinales (GIST) son las neoplasias mesenquimales más frecuentes del tracto gastrointestinal. Históricamente, estas lesiones fueron clasificadas como leiomiomas o leiomiosarcomas; sin embargo, el verdadero origen de los GISTs es una célula madre mesenquimal pluripotencial, programada para diferenciarse en la célula intersticial de Cajal. Estos avances han conducido a la clasificación de los GISTs como una entidad separada de los tumores de músculo liso. Presentamos el caso de un paciente de 39 años con el diagnóstico de un tumor del estroma gastrointestinal del intestino delgado, hecho infrecuente en la presentación clínica de esta rara entidad. Se analizan las manifestaciones clínicas, la epidemiología y la importancia de la inmunohistoquímica en el diagnóstico de estos tumores. Por último, se señala el diagnóstico diferencial y la terapéutica, resaltándose la importancia de la cirugía y el uso del Imatinib (Glivec) en estos procesos.
ABSTRACT
Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract. Historically, these lesions were classified as leiomyomas or leiomyosarcomas, but the actual cell of origin of GISTs is a pluripotential mesenchymal stem cell programmed to differentiate into the interstitial cell of Cajal. These advances have led to the classification of GISTs as an entity separate from smooth muscle tumors. The case of 39-year-male patient with the diagnosis of gastrointestinal stromal tumor of the small intestine is presented. The clinical manifestations, epidemiology and immunohistochemical diagnostic features are discussed. Finally, differential diagnosis and therapeutics are exposed. We stood out the importance of the surgical management and Imatinib (Glivec) in these tumors.
