Novas recomendações para prevenção de endocardite infecciosa na odontologia: comparação entre as recomendações de profilaxia antibiótica preconizadas pela American Heart Association de 1977 a 2007 (última revisão) / New recomendations for the prevention of infective endocarditis in dentistry: comparison among antibiotic

RESUMO

A American Heart Association tem feito recomendações para a prevenção de Endocardite Infecciosa (EI) por mais de 50 anos. O crescente desenvolvimento de novos fármacos afetou a escolha dos medicamentos na tentativa de se recomendar um fármaco cuja dose fosse a menor e mais eficiente possível. Entretanto, desde a última revisão, em 1997, muitas autoridades e sociedades, bem como as conclusões de vários estudos publicados, têm questionado a eficácia da profilaxia antimicobriana para previnir EI em pacientes que se submetem a procedimentos dentais, gastrointestinais ou genito-urinários, sugerindo uma nova revisão. Este artigo tem como objetivo mostrar essas profundas mudanças ocorridas no documento de 2007, revisando também a quimioprofilaxia preconizada em documentos anteriores.

ABSTRACT

The American Heart Association has made recommendation for the prevention of Infective Endocardits (EI) for over 50 years. The continuing improvement and formulations of new drugs affected the treatment choices in order to recommend a drug that would need the lowest and the most efficient sose. However, since last revision, in 1997, many authorities ando societies, as well as the published studies, have questioned the efficacy of antimicrobial prophylaxis to prevent IE in patients who undergo a dental, gastrointestinal, or genitourinary tract procedure and have suggested that the AHA guidelines should be revised. The aim of this article is to show the profund changs that occurred in the 2007 document, also revising the chemoprophylaxis recommended in the previous documents.
