Comparação dos efeitos dentoesqueléticos decorrentes do tratamento realizado em duas fases (comaparelho de Herbst e aparelho fixo pré-ajustado) em adolescentes com retrognatismo mandibular / Comparison of the dentoalveolar-skeletal effects of two-phase treatment (with the Herbst and preadjusted fixed appliance) in adolescents with mandibular retrognathism

RESUMO
Neste estudo prospectivo foram avaliadas as mudanças dentoesqueléticas decorrentes do tratamento da má-oclusão de Classe II, div. 1ª de Angle e retrognatismo mandibular, tratados em duas fases de 13 meses (I, fase ativa de tratamento com Herbst e II, fase pós-Herbst). As telerradiografias laterais, de 20 adolescentes consecutivos, obtidas em três tempos (T1-Início, T2-no termino da fase ativa e T3-término da fase pós-Herbst) foram traçadas e analisadas estatisticamente. Os resultados mostraram que de TI a T2 tanto a maxila quanto a mandíbula se projetaram para anterior; porém com incrementos significativamente maiores na mandíbula, permitindo um ajuste maxilomandibular; os incisivos superiores retroinclinaram e os inferiores protruíram; os molares superiores distalizaram e os inferiores mesializaram e o trespasse horizontal diminuiu. De T2 a T3, a maxila continuou a projetar-se para anterior e a mandíbula apresentou um incremento menor; os incisivos superiores permaneceram estáveis e os inferiores lingualizaram; os molares superiores e os inferiores mesializaram. Comparando T1 a T3 observou-se correção da má-oclusão, em decorrência da soma das mudanças dentárias e esqueléticas. Pode-se concluir que as mudanças caracterizaram diferencialmente as duas fases de tratamento, sendo que na Fase I houve um maior incremento do crescimento mandibular e mudanças dentárias que sobrecorrigiram a má-oclusão, enquanto que na Fase II houve uma desaceleração do crescimento em combinação com recidiva das posições dos molares e incisivos inferiores, porém, não comprometendo as metas ideais do tratamento. O tipo facial foi preservado.
ABSTRACT
The aim of this prospective study was to observe dental-skeletal changes occurring after the treatment of the Angle Class II/1 malocclusion and mandibular retrognatism. The patients were treated in two 13-month phases. Phase I was done with the Herbst appliance and Phase II was the period afier Herbst removal. Lateral cephalograms from 20 consecutive patients were obtained in 3 different times: T1, at the beginning of the treatment, T2 at the end of Herbst treatment and T3, at the end of the post Herbst period. The results showed thatfvom T1 to T2 either the maxilla or the mandible wereprojected anteriorly with a significant major increase in the mandible allowing the maxillarymandibular adjustment. The upper incisors were inclined distally while the lowers were protruded. Upper molars were distalized while the lowers ones mesialized and the overjet was diminished. From T2 to T3 the maxilla continued to grow anteriorly. The mandible grew less than in phase I. The upper incisors stayed stable while the lowers were moved lingually. Upper and lower molars showed a mesial movement. Comparing TI with T3 a correction of the malocclusion was observed due to thesum of the skeletal and dental changes. It could be concluded that the changes characterized differently the two phases of the treatment: during phase I there was a bigger mandibular growth and dental changes that overcorrected the malocclusion while during phase II there was a decrease of the growth rate and a partial relapse of the position of the lower incisors and molars hut without jeopardizing the ideal goals of the treatment. Facial type was preserved.
