Reconstrução parcial da língua com retalho microcirúrgico / Partial tongue reconstruction with microsurgical flap

RESUMO

Introdução: A reconstrução em cirurgia oncológica de cabeça e pescoço teve grande avançocom o uso de retalhos microcirúrgicos. Objetivo: O objetivo desse trabalho é relatar acasuística de reconstrução da região lateral da língua e avaliar se os retalhos microcirúrgicospossibilitaram reconstrução funcional da mesma. Método: No período de novembro de2006 a julho de 2009, oito pacientes tiveram como diagnóstico carcinoma espinocelular(CEC) na região lateral da língua (estadiamento T1 a T3). A indicação cirúrgica foi ahemiglossectomia com esvaziamento cervical e reconstrução com retalho microcirúrgico.Os retalhos utilizados foram o antebraquial (n=7) e o ântero-lateral da coxa (n=1). Foramavaliadas duas funções da língua a fala e a deglutição. Resultados: Não houve perdastotais ou parciais de retalhos nesta casuística e observou-se um bom resultado funcional emsete pacientes. Conclusão: O uso de retalhos microcirúrgicos preenche os pré-requisitospara reconstrução funcional da língua.

ABSTRACT

Background: The reconstruction for head and neck cancer had a notorious upgrade withthe microsurgical flap transplantation. Objetive This article purpose is present the experiencein lateral tongue reconstruction and evaluate its functional recovery. Method:From November 2006 to July 2009, eight patients had espinocelular carcinoma diagnosedin the hemitongue (T1 to T3 stage). Hemiglossectomy with neck dissection followed bymicrosurgical flap reconstruction was chosed as therapy. The flaps employed were the antebraquial(n=7) and anterolateral thigh flap (n=1). The functions evaluated were speechand deglutition. Results: All the flaps survived and intelligible speech with regular oraldiet was noted in all patients except one. Conclusions: The microsurgical flaps fulfil thecriteria for functional reconstruction of the tongue.
