Avaliação da força muscular periférica em pacientes asmáticos graves / Peripheral muscle strength evaluation in severe asthma patients

RESUMO

A asma é uma doença inflamatória crônica resultante de uma interação genética, exposição ambiental e outros fatores específicos que levam ao desenvolvimento e à manutenção dos sintomas. O objetivo do trabalho é avaliar a força muscular periférica em pacientes asmáticos graves. Foram avaliados 20 pacientes com asma grave e 12 indivíduos saudáveis do sexo feminino. Os pacientes estavam sob tratamento médico-ambulatorial há pelo menos seis meses no Ambulatório do Hospital do Servidor Publico de São Paulo- IASMPE, com quadro clínico estável durante três meses e com terapia medicamentosa diariamente otimizada. Foram realizados os seguintes procedimentos anamnese, espirometria, resistência máxima (1 RM) e teste de resistência. Vinte pacientes foram inicialmente triados no estudo, dos quais seis não conseguiram concluir por exacerbação ou desistiram da avaliação. A força muscular periférica determinada por 1 RM não apresentou diferença significante (65,46±20,63 vs 75,22±11,51) entre os grupos asma e controle, respectivamente. Já em relação à resistência, observou-se uma diferença significativa de 0,045 entre os grupos. Os resultados mostraram uma diferença significativa na resistência muscular periférica, o que não ocorreu com a força, sugerindo que esses pacientes provavelmente apresentam esta perda por uso de glicocorticóides e pelo sedentarismo.

ABSTRACT

Asthma is a chronic infl ammatory disease, resultant from a genetic interaction, environmental expositionand others specific factors that lead to the symptoms development and maintenance. The objective of the studyis to evaluate the peripheral muscles strength in severe asthma patients. It was evaluated 20 patients with severe asthma and 12 healthy female subjects. The patients were under medical treatment, outpatient for at least six months at the Hospital Servidores Publicos de São Paulo-IASMPE with clinical stable for three months and with drug therapy daily optimized. It were performed the following procedures anamnesis, spirometry, maximum strength (1 RM) and endurance test. Results: Twenty patients were initially screened in the study. Six patients failed to complete the study or dropped by exacerbation of the assessment. Peripheral muscle strength determined by 1 RM showed no signifi cant difference (65.46 ± 20.63 vs 75.22 ± 11.51 Kg) between the asthma and control groups respectively. Regarding to the resistance, there was a significant difference of 0.045 between the groups. The results showed a signifi cant difference in peripheral muscle resistance, which did not occurred with strength, suggesting that these patients probably present such loss by the use of glucocorticoids and sedentariness.
