Propuesta de algoritmo para mejorar la atención integral al paciente pediátrico con enfermedad oncológica en Atención Primaria de Salud / Algorithm proposal to improve comprehensive health care to pediatric patient with oncological disease in Primary Health Care

RESUMEN

Se realizó una investigación descriptiva transversal de enero a diciembre del 2008 con el objetivo de determinar la satisfacción con la atención recibida según la percepción del familiar del paciente pediátrico con enfermedad oncológica, y luego proponer un algoritmo para mejorar la atención a estos pacientes en la provincia de Ciego de Ávila. La muestra estuvo conformada por la totalidad de los responsables o tutores, con criterio de inclusión y firma del consentimiento informado. Se aplicó una entrevista estructurada y validada. Los datos se distribuyeron en tablas, comentarios y esquema. Solo el 21,2 por ciento estaba satisfecho con la atención recibida. Teniendo en cuenta los resultados, se diseñó un algoritmo para mejorar la atención al paciente pediátrico con enfermedad maligna en la Atención Primaria de Salud en cinco etapas (etapa I: sospecha de cáncer infantil, etapa II: paciente con tratamiento, etapa III: paciente oncopediátrico con cáncer en remisión, etapa IV: paciente oncopediátrico sobreviviente, etapa V: paciente oncopediátrico con enfermedad terminal). Este algoritmo se propone para su generalización en la provincia, con evaluación posterior de los resultados.

ABSTRACT

A cross-sectional descriptive research was carried out from January to December, 2008 in order to determine satisfaction with the attention received, according to the perception of the family of pediatric patient with oncological disease, and then propose an algorithm to improve care to these patients in Ciego de Ávila Province. The sample was made up of all those responsible people or guardians criteria for inclusion and signature of informed consent. A structured and validated interview was applied. The data were distributed in tables, comments and schema. Only 21.2 percent was satisfied with the received attention. Taking into account the results, an algorithm is designed to improve the primary health care attention of pediatric patients with malignant disease in five stages (stage I: suspicion of childhood cancer, stage II: patient with treatment, stage III: oncopediatric patient with cancer remission, stage IV: surviving oncopediatric patient, phase V: oncopediatric patient o with terminal illness). This algorithm is proposed for its generalization in our province, with post- evaluation of the results.
