RESUMO
A avaliação da qualidade de vida em pacientes com neoplasias das vias aerodigestivas superiores proporciona um melhor entendimento do impacto da doença na vida dos indivíduos, e ajuda na escolha terapêutica em cenários onde diferentes modalidades de tratamento podem ser utilizados sem diferença significativa no controle da doença e nas taxas de sobrevida. Porém, para o estudo da qualidade de vida, o instrumento escolhido deve ter sua validade psicométrica bem estabelecida. Além da avaliação da qualidade de vida, o estudo da ocorrência de incapacidade para o trabalho também poderia ajudar e entender os efeitos socioeconômicos da doença. Como objetivos, este estudo visou realizar a validação em Português do questionário de avaliação de qualidade de vida da Universidade de Washington, estudar a qualidade de vida dos pacientes tratados em nosso meio, e avaliar a ocorrência de incapacidade para o trabalho e suas correlações. Um estudo prospectivo de uma série consecutiva de 109 pacientes tratados por carcinoma epidermóide das vias aerodigestivas superiores foi realizado para estabelecer a validação psicométrica do instrumento de pesquisa em nossa língua, e um estudo transversal de uma série de 344 pacientes tratados na instituição com pelo menos dois anos de sobrevida livre de doença foi realizado para se estudar a qualidade de vida dos sobreviventes em longo prazo, e avaliar a ocorrência de incapacidade para o trabalho neste grupo de pacientes...A avaliação da qualidade de vida nos sobreviventes em longo prazo foi considerada como boa ou excelente pela maioria dos pacientes (78,5%)... A ocorrência de incapacidade para o trabalho neste estudo foi detectada em um terço dos pacientes (32,9%), sendo evidenciado que o estádio avançado dos tumores, o consumo de álcool e o baixo nível sociocultural foram os principais fatores de risco...(AU)
ABSTRACT
The quality of life evaluation in patients with upper aerodigestive tract cancer proportionates a better understanding of the disease impact on the individual's life, and it helps the treatment choice in scenarios where different treatment modalities can be used without significant differences in the disease control and overall survival. However, for a quality of life study, the instrument should have its psychometric validation well recognized. Besides quality of life evaluation, the occurrence of work disability could also help to understand the socioeconomic effects of the disease. The purposes of this study was to validate the University of Washington quality of life questionnaire into Brazilian-Portuguese language, to study the quality of life of patients treated in the institution, and to evaluate the occurrence of work disability and their correlations. A prospective study of a consecutive series of 109 patients treated for upper aerodigestive tract squamous cell carcinoma was performed to establish the psychometric validation of the survey instrument in our language, and a cross-sectional study of a series of 344 patients treated in the institution with at least two years of disease-free survival was performed to study the quality of life of long-term survivals, and to evaluate the occurrence of work disability in this group of patients. The translation and cultural adaptation of the University of Washington questionnaire as well as its psychometric validation were well established, with good results regarding the reliability (internal consistency with Cronbach's alpha of 0.744, and test-retest reliability with intraclass correlation coefficient of 0.882) and construct validity (with statistically significant relationships with the SF-36 generic quality of life questionnaire and with the HAD questionnaire to evaluate anxiety and depression), showing that the questionnaire can be safely used in our country. The quality of life evaluation in long-term survivals was considered good or excellent in most patients (78.5%), being the advanced tumors and the combined treatment considered the factors with the greatest negative impact on the individual's quality of life. In this study, the occurrence of work disability was detected in one third of patients (32.9%), with the advanced clinical stage, alcohol consumption, and low socioeducational level being considered the main risk factors for such disability (AU)
