Sorriso gengival: uma opção de tratamento / Gummy smile: a treatment option

RESUMO

O presente estudo teve por objetivo abordar o diagnóstico e tratamento do sorriso gengival em suas múltiplas etiologias. Como a busca pela melhora estética do sorriso tem levado muitos pacientes aos consultórios odontológicos, o sorriso gengival é um aspecto que causa grande preocupação por parte dos dentistas em geral. Os autores avaliados concordam que a harmonia dos tecidos moles com os dentes é o aspecto mais importante no sorriso, porém, no sorriso ideal a borda inferior do lábio superior descansa sobre ou até 2 mm acima da margem gengival dos incisivos superiores e a curvatura desses é coincidente com a curvatura do lábio inferior. Uma maior exposição gengival é geralmente tratada como indesejável pelos dentistas. Entre as causas do sorriso gengival estão excesso vertical de maxila, hiperatividade do lábio superior, erupção passiva alterada e sobre-erupção dos incisivos superiores. As formas existentes para a correção do excesso de exposição gengival ao sorrir estão relacionadas à causa e à gravidade do problema e à expectativa do paciente, sendo o diagnóstico diferencial a chave para o sucesso do tratamento que pode requerer tratamento cirúrgico periodontal, ortodontia, cirurgia ortognática, procedimentos relacionados à cirurgia plástica ou mesmo a combinação de vários desses recursos para a obtenção do resultado desejado.

ABSTRACT

The present study had as aim to approach the diagnosis and treatment of gummy smile in its multiple causes. As the search for the smile’s aesthetic improvement takes many patients to the dentists’ office, the gummy smile is an aspect that concerns a lot clinicians. The authors agree that the harmony between soft tissues and the teeth is the most important aspect in a smile, however, on the ideal smile, the inferior border of the upper lip rests at the line or up to 2 mm above the gingival margin of the superior incisors and their curvature is coincident with the curvature of the inferior lip. The dentists usually consider a larger gingival exhibition as undesirable. Among the causes of the gummy smile there are vertical maxillary excess, upper lip hyperactivity, altered passive eruption and over-eruption of the superior incisors. The existent forms for the correction of the excess of gingival display on smile are related to the cause, severity of the problem and patient’s expectation. The differential diagnosis is the key for the success of the treatment that may need periodontal surgery, orthodontics, ortognatic surgery, plastic surgery procedures or even the combination of several of those resources for a result of success.
