RESUMO
Após o infarto agudo do miocárdio podem ocorrer complexas alterações da arquitetura ventricular, envolvendo tanto o ventrículo infartado como o ventrículo contralateral. Recentemente estas alterações passaram a ser designads como remodelação ventricular (RV) pós-infarto. Do ponto de vista clínico, a remodelação pode estar associada a pior prognóstico, após o insulto isquêmico. Assim, foi verificado que este processo se associa a ruptura ventricular, piora funcional, arritmias e formação de aneurismas. Outro fator relevante refere-se ao fato de que a evolução do processo de remodelação pode ser modificada por meio de diversas intervenções terapêuticas. Entre as estratégias para atenuar a RV destacam-se a permeabilidade de artéria relacionada ao infarto, agentes inotrópicos, bloqueadores beta-adrenérgicos, nitratos e os inibidores de enzima conversora da angiotensina.
ABSTRACT
After myocardial infarction, complex alterations in ventricular architecture can be observed, which could involve both, the infarcted and noninfarcted ventricle. Recently, these alterations started to be referred as postinfarction ventricular rremodeling (VR). Under the clinical view point, ventricular remodeling can be associated to a poor prognosis, after the ischemic event. Thus, it was verified that this process is associatted to ventricular rupture, functioinal impairment, arrhytmias and aneurism formation. Another important factor related to VR, is that this process can be modified by different therapeutic maneuvers. using these medical strategies, one can attenuate the VR process by means of vistoration of coronary artery patency in the infarcted area; intropic agents; beta-adrenergic blockers; nitrates and angiotensin converting enzime inhibition.
