Prevalência do aleitamento materno exclusivo em um município serrano do Rio Grande do Sul, Brasil / Prevalence of exclusive breastfeeding in south Brazil

RESUMO

Introdução: Investigar a prevalência do aleitamento materno exclusivo em crianças menores de um ano em Bento Gonçalves/RS e avaliar as possíveis associações entre aleitamento materno exclusivo e variáveis socioeconômicas, ambientais reprodutivas e vínculo mãe-bebê. Metodologia: Estudo transversal, com mães e crianças menores de um ano de idade. Os dados foram obtidos mediante entrevistas com perguntas diretas, seguidas pela observação de indicadores de vínculo mãe-bebê, durante Campanha Nacional contra a Poliomielite de 2008. Foi utilizado um modelo hierarquizado para as variáveis preditoras e analisados através da regressão de Cox. Resultados: Foram incluídas 876 díades. Destas, 623 (71%) receberam leite materno nas últimas 24 horas. A prevalência do aleitamento materno exclusivo nos menores de 6 meses de idade foi de 55,1% (n=233). A mediana de aleitamento materno exclusivo foi de 63 dias. As variáveis que na análise multivariada apresentaram significância estatística (p < 0,05) com o desfecho de aleitamento exclusivo foram: trabalho materno, uso de chupeta, mamadeira e não participação em grupo de gestantes. Conclusões: A prevalência do aleitamento materno exclusivo, no município de Bento Gonçalves (RS), mostra-se abaixo da preconizada pela Organização Mundial da Saúde. As variáveis com associação significativa na análise multivariada são passíveis de intervenções. Procedimentos de prevenção primordial, como participação em grupo de gestantes, evitar uso de chupeta e não introdução de outros alimentos precocemente deve ser estimulada.

ABSTRACT

Introduction: To investigate the prevalence of exclusive breastfeeding in children under one year of age in Bento Gonçalves/RS, and to evaluate potential associations between exclusive breastfeeding and socioeconomic, reproductive environmental variables, and mother-child bonding. Methods: Cross-sectional study with mothers and their children under one year of age. The data were obtained through interviews with direct questions, followed by theobservation of mother-child bonding indicators, during the 2008 National Campaign against Poliomyelitis. A hierarchical model for the predictive variables was used and analyzed through Cox regression. Results: A total of 876 dyads were included. Among these, 623 (71%) infants had been breastfed in the last 24 hours. The prevalence of exclusive breastfeeding among infants under 6 months of age was 55.1% (n=233). The median of exclusive breastfeeding was 63 days. In the multivariate analysis, the variables which presented statistical significance (p < 0.05) with the outcome exclusive breastfeeding were: mother work, use of pacifier, baby bottle, and non-participation in pregnant women support group. Conclusions: The prevalence of exclusive breastfeeding in the municipality of Bento Gonçalves (RS) is below the one recommended by the World Health Organization. The variables with significant association in the multivariate analysis are potentially subject to interventions. Procedures of primary prevention, such as attending a pregnant women support group, avoiding the use of pacifiers, and postponing the introduction of other foods, should be encouraged.
