Esclerose lateral amiotrófica e o tratamento com células-tronco: [revisão] / Amyotrophic lateral sclerosis and stem cells treatment: [review]

RESUMO

JUSTIFICATIVA E OBJETIVOS: A esclerose lateral amiotrófica (ELA) é uma doença neurodegenerativa que afeta os neurônios motores inferiores e superiores e que, frequentemente, encontra-se associada às alterações bulbares e no trato piramidal. Apesar de existirem algumas variações nas manifestações clínicas, no padrão de progressão da doença e na expectativa de vida após o início dos sintomas, o diagnóstico é realizado através de exame clínico e, muitas vezes, é dado por exclusão. Para o tratamento da ELA existe, até o momento, apenas um medicamento aprovado pelo FDA, o riluzol, sendo que outros fármacos são frequentemente associados à terapia no intuito de amenizar os sintomas e melhorar a qualidade de vida do paciente. O objetivo deste estudo foi atualizar os conhecimentos referentes à doença, bem como discutir os protocolos de tratamento com células-tronco mais recentes para ELA. CONTEÚDO No sentido de se procurar um tratamento efetivo para a ELA, vários pesquisadores vêm utilizando células-tronco devido ao potencial destas de se diferenciarem em diferentes tipos de células nervosas in vivo e in vitro e que quando transplantadas no cérebro, são capazes de sobreviver, migrar e integrar, apresentando uma funcionalidade ativa. Assim, foi realizado um levantamento dos principais estudos que avaliaram o uso das células-tronco como tratamento da ELA. CONCLUSÃO: Enquanto todas as indagações sobre o uso das células-tronco não forem elucidadas; e não forem propostas terapias que garantam a sua eficácia e segurança para os pacientes, novas pesquisas ainda são necessárias a fim de utilizar a terapia celular como alternativa de tratamento da ELA.

ABSTRACT

BACKGROUND AND OBJECTIVES: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease which affects upper and lower motor neurons and is frequently associated to alterations in bulbar and pyramidal tract. Although there are variations on clinical manifestations, on disease progression and life expectation after the first symptoms, diagnosis is usually made by clinical evaluation and exclusion. At the moment there is only one medicament approved by FDA, riluzole, which is frequently associated with others to decrease symptoms and improve patient's life quality. The present revision has the objective to bring up to date the knowledge about the ALS, as well discuss the more recent stem cells protocols for ALS treatment. CONTENTS As a tentative to find an effective treatment for ALS many researches are using stem cells due their capacities to differentiate in many neuronal cell types in vivo and in vitro and when transplanted into the brain, these cells are capable of survive, migrate and integrate showing a functional activity. The present work shows the main studies that had evaluated the use of the stem cells as treatment of ALS. CONCLUSION: While all the investigations about the stem cells use aren't elucidated and there aren't proposals therapies that guarantee the effectiveness and patient's security, new research are still necessary in order to use the cellular therapy as alternative of treatment of ELS.
