Frecuencia del fenotipo "cintura hipertrigliceridémica' y su asociación con el síndrome metabólico en adultos con sobrepeso y obesidad / The "hypertriglyceridemic waist" phenotype and its association with the metabolic syndrome in overweight and obese adults

RESUMEN

Objetivo. Identificar la frecuencia del fenotipo "cintura hipertrigliceridémica" (CHT) en adultos con sobrepeso y obesidad y determinar su asociación con el síndrome metabólico (SM). Material y Métodos. Estudio retrospectivo de casos y controles en una población de 426 pacientes adultos con sobrepeso y obesidad que acudieron a la Unidad de Peso Sano del Hospital Nacional Arzobispo Loayza de Lima, entre enero 2004 y junio 2007. Se trabajó con los datos recopilados (medidas antropométricas y pruebas bioquímicas). Resultados. La muestra estuvo constituida por un 90% de mujeres, con una edad promedio de 46 años (18-79 años). Se determinó 274 casos de SM (64,3%) y 152 controles. El fenotipo CHT se presentó en un 41,8% y cuando este fue modificado(disminuyendo el punto de corte para triglicéridos mayor o igual que 150 mg/dL) fue 56,3%, y presentó una asociación moderadamente fuerte al compararlo con la presencia de SM (Pearson = 1,68, p = 0,00, V de Cramer = 0,629); además, sensibilidad de 79% y especificidad de 85%. Alteraciones en el nivel de triglicéridos y en la circunferencia de cintura evaluados independientemente mostraron OR = 21,7 (IC 95%= 12,8-36,8) y 3,7 (IC 95% = 0,9-15), respectivamente. Conclusión. La frecuencia del fenotipo CHT modificado fue del 56,3% entre los adultos con sobrepeso y obesidad y se encontró una buena asociación estadística entre el fenotipo CHT modificado y el SM lo que ratifica su aplicabilidad como prueba de despistaje de riesgo cardiovascular.

ABSTRACT

Objective. To identify the frequency of the "hypertriglyceridemic waist" (HTW) phenotype in overweight and obese adults, and its association with the metabolic syndrome (MS). Material and methods. A case control, retrospective studio was performed in 426 overweight and obese adult patients of the Healthy Weight Unit of the Arzobispo Loayza National Hospital, Lima, between January of 2004 and June of 2007, using the recorded data (anthropometric measures and biochemical tests). Results. The 90% of the sample was women, with an average age of 46 year-old (18-79 year-old); 274 cases (64,3%) were identified as the MS and the control group was composed by 152 patients. The HTW phenotype was detected in 41,8% and it was 56,3% when triglycerides cut off point was modified to more or equal than 150 mg/dl and it has a strong association with the presence of the MS (Pearson = 1,68, p = 0,00, Cramer's V = 0,629), with a 79% sensibility and a 85% specificity. Triglyceride abnormal levels and higher waist circumference showed OR= 21,7 (IC 95%= 12,8-36,8) and 3,7 (IC 95%= 0,9-15) respectively.Conclusion. The modified HTW phenotype frequency was 56,3% among overweight and obese adults and it has a strong association with the MS which confirm its applicability as a screening test of cardiovascular risk.
