RESUMO
Trata-se do relato de caso de uma gestante atendida no setor de Pronto Atendimento de Obstetrícia do Hospital Júlia Kubitschek com quadro de descolamento prematuro de placenta (DPP). Havia relato de sangramento genital de grande monta, dor abdominal e rigidez da parede uterina. Não foram detectados batimentos cardíacos fetais ao sonar. Com isso, a suspeita de DPP foi prontamente levantada, o que gerou a internação imediata. Foi feita cesariana de emergência, com retirada de um feto natimorto. A paciente evoluiu com sangramento uterino importante no pós-operatório imediato, com fortes repercussões hemodinâmicas e necessidade de outra abordagem para realização de histerectomia subtotal. É um caso que evidencia a grande influência do DPP na morbimortalidade materno-fetal.
ABSTRACT
This is the case report of a patient treated at the Emergency Services sector of obstetrics at Júlia Kubitscheck hospital with signs of a placental abruption typical case (premature separation of placenta). There was report of intense vaginal bleeding, abdominal pain and uterine hypertonia. There was no fetal heartbeat noticed by sonar. Thus the suspicion of premature placenta abruption was promptly raised, which led to emergency cesarean section, with the removal of a stillborn fetus. The patient developed significant uterine bleeding after the cesarean section with strong homodynamic repercussion and need for another approach to perform subtotal hysterectomy. It is a case that demonstrates the great influence of the placental abruption in maternal-fetal morbidity and mortality.
