RESUMEN

El establecimiento de un adecuado diagnóstico y plan de tratamiento en pacientes con desórdenes temporomandibulares (DTM) y dolor orofacial (DOF) está estrictamente relacionado con la correcta recolección de la información asociada con los diferentes signos y síntomas del paciente. Una de las principales causas por las cuales los tratamientos en DTM/DOF fallan se debe aun diagnóstico incorrecto, lo cual lleva al establecimiento de malos manejos y de hecho a consecuencias que podrian ser devastadoras para los pacientes. Por estas razones y con el propósito de afrontar la complejidad inherente del DOF en la práctica clínica, el proceso diagnóstico debe ser sistemático y soportado por niveles adecuados de contabilidad y validez, que permitan al clínico tener una mayor certeza en el establecimiento del diagnóstico diferencial. El proposito de este artículo es presentar un protocolo de evaluación, que muestre de manera lógica y la incertidumbre al momento de establecer el diagnóstico diferencial y a su vez incrementar las posibilidades de éxito en el manejo de DTM/DOF.

ABSTRACT

The establishment of an adequate diagnosis and treatment plan in a patient with temporomandibular disorders (TMD) and orofacial pain (OFP) is strictly related to the correct recollection of the information associated with the different signs and symptoms of the patient. One of the principal reasons for failure of TMD and OFP treatment is incorrect diagnosis of the patient which lead to the establishment of bad management and consequences that could be truly devastating for the patients. For these reasons and to confront the inherent complexity of orofacial pain in clinical practice, the diagnostic process must be systematic and supported with good levels of reliability and validity which will allow the clinician to have a greater certainty in the establishment of the differential diagnosis. The purpose of this article is to present a protocol for evaluation that illustrates in a logical and structured way the different steps in the evaluation process. This also allows the clinician to reduce the level of uncertainty in the differential diagnosis and in the same way, increase the level of success in the management of TMD/ OFP.
