Aspectos controversiales en relación al manejo de los terceros molares asintomáticos / Controversial issues regarding the management of asymptomatic wisdom teeth

RESUMEN

La extracción de los terceros molares (3M), erupcionados o retenidos, es el procedimiento quirúrgico más frecuentemente realizado en cirugía buco maxilofacial (CBMF). No existen dudas sobre su indicación de extracción cuando son la causa de dolor, infección o algún otro síntoma o signo. Pero, existe aún controversia, con respecto al manejo de aquellos 3M retenidos o semierupcionados asintomáticos. Algunos clínicos, consideran que su extracción profiláctica (EP) se debe realizar para evitar que ocasionen problemas. Sin embargo, otros opinan, que el control clínico y radiográfico frecuente, es la opción más adecuada.En este trabajo, se revisó la literatura para analizar alguna de las indicaciones más frecuentemente mencionadas, con la intención de discutir y clarificar alguno de los aspectos controversiales respecto al manejo de los 3M asintomáticos.

ABSTRACT

The extraction of third molars (3M) erupted or impacted, is the most frequent procedure done by the oral and maxillofacial surgeon. There is no doubt in regard of its indication when they cause pain, inflammation or other symptoms or signs. However, there are still controversies with regard the management of asymptomatic impacted or partially erupted 3M. One ofthe opinions is that the prophylactic removal (PR) is recommended in order to prevent any problems. But on the other hand, some clinicians’ advice clinical and radiographical follows up.In this paper, a review of the literature was done in order to analyse some of the most frequent indications for clarification and discussion of such controversial aspects regarding the management of the 3M asymptomatic.
