Vulnerabilidade e exposição a marcadores sorológicos dos Vírus da Imunodeficiência Humana, hepatites B e C, vírus linfotrópico de células T humanas e sífilis em pacientes psiquiátricos internados em hospital público / Vulnerability and exposure to serological markers of Human Immunodeficiency Virus, hepatitis B and C, human T cell lymphotropic virus and syphilis among psychiatric patients admitted to a public hospital

RESUMO

Com o objetivo de identificar, em pacientes psiquiátricos internados, a vulnerabilidade expressa por meio dos comportamentos e situações de risco, e as taxas de exposição aos marcadores sorológicos dos virus da imunodeficiência humana (VIH), hepatites B e C, vírus linfotrópico de células T humanas (VLTH) e da sífilis, foram examinados, entre junho de 1997 a outubro de 2000, todos os pacientes internados em uma enfermaria masculina do Instituto Raul Soares (IAS), hospital psiquiátrico público, integrante da Fundação Hospitalar do Estado de Minas Gerais (FHEM)G). De um total de 751 pacientes, 422 foram identificados com algum tipo de risco por meio do exame clínico. Os principais riscos identificados foram o não-uso ou uso irregular do preservativo, uso de fumo, álcool e drogas. 314 pacientes realizaram exames sorológicos, com taxas positivas de 19,7% de anti-HBc, 1,6% de HBsAg, 5,7% de anti-HCV, 1,6% de anti- HIV, 0,6% de anti-HTLV I/II e 7,0% de VDRL reativo, com 3,8% de FTA-ABS ou equivalente positivos. O exame clínico, a identificação de riscos comportamentais e situacionais e as taxas de exposição registradas foram importantes para abordagens preventivas, terapêuticas e para o maior conhecimento das inter-relações entre pacientes com sofrimento mental e estas doenças infecciosas e transmissíveis.

ABSTRACT

Patients admitted to a male ward of Raul Soares Institute, a public psychiatric hospital in Minas Gerais, from June 1997 to October 2000, were examined in order to identify the vulnerability of hospitalized psychiatric patients to the serologic markers of human immunodeficiency virus (HIV), hepatitis B and C, human T cell lymphotropic virus (HTLV) and syphilis. Clinical evidence revealed some risks in 422 out of 751 patients. The main risks detected were irregular or no use of a condom, smoking and alcohol and drug consumption. Serologic tests were applied in 314 patients. The following positivity rates were detected 19.7% anti-HBc, 1.6% HBsAg, 5.7 % anti-HCV, 1.6% anti HIV, 0.6% anti-HTLV I/II, and 7.0% reactive VDRL, with 3.8% positive FTA-ABS or equivalents. The clinical examination, the identification of behavioral and situation-based risks and the rate of exposure recorded were important for preventive approaches, treatment and the construction of knowledge about the relationship among subjects with mental illness and these transmissible diseases.
