Sofrimento psíquico interferindo no desejo materno de cuidar do filho internado na unidade de terapia intensiva pediátrica / Psychological suffering interfering in the maternal desire of taking care of a child hospitalized in the pediatric intensive care unit

ABSTRACT

Introduction: In order to take care of a hospitalized child a mother abdicates of her tasks, routines and family coexistence. The objective of the study was to apprehend aspects of a mother’s living while her child stays in the Pediatric Intensive Care Unit (PICU). Methodology: A descriptive study with qualitative approach that used the Assistant Convergent Research (ACRA) as methodological referential and the Transcultural Theory of Caring as theoretical referential. Participated in the study six mothers who had their children in the PICU of a University Hospital located in the Northwest of Paraná State. Data were collected from January to May 2007, through semi-structured interviews, orientation and participant observation. For analysis and interpretation of the data, the four generic processes of ACRA were used. Results: Belief and cultural values delineate the form how people take care and conceive caring, including during hospitalization. Three categories emerged Maternal feelings facing hospitalization of a child; Family loss of orientation caused by the internment; and Deficit of the desire of taking care. Implications for Nursing A child's hospitalization unchains in the family changes and sufferings that reflect directly upon the mother, for culturally, being the responsible person for the child care, including during hospitalization. This increases her psychological suffering, in such a way that she can feel impotent, unable and without will of taking care of her child in the hospital atmosphere. Health professionals, with views to an integral assistance, need to understand this limitation, support the mother in her needs, and help her to overcome this condition.

RESUMO

Introdução: Para cuidar do filho hospitalizado a mãe abdica de seus afazeres, rotinas e convivência familiar. O objetivo do estudo foi apreender aspectos da vivência materna durante a internação do filho na Unidade de Terapia Intensiva Pediátrica (UTIP). Metodologia: Pesquisa descritiva com abordagem qualitativa, que utilizou a Pesquisa Convergente-Assistencial (PCA) como referencial metodológico e a Teoria Transcultural do Cuidado como referencial teórico. Participaram do estudo seis mães que tiveram seus filhos internados na UTIP. de um Hospital Escola localizado na Região Noroeste do Paraná. Os dados foram coletados no período de janeiro a maio de 2007, por meio de entrevistas semi-estruturadas, orientação e observação participante. Para análise e interpretação dos dados, foram utilizados os quatro processos genéricos de PCA. Resultados: As crenças e valores culturais delineiam a forma como as pessoas cuidam e concebem o cuidado, inclusive durante a hospitalização. Emergiram três categorias Sentimentos maternos diante da internação do filho na UTIP; Desestruturação familiar causado pela internação; e Déficit do desejo de cuidar. Implicações para a enfermagem A hospitalização da criança desencadeia na família alterações e sofrimentos que refletem diretamente na mãe, por ser esta culturalmente a pessoa responsável pelo cuidado do filho inclusive durante a internação. Isto aumenta ainda mais o seu sofrimento psíquico, de tal forma que ela pode se sentir impotente, incapaz e sem vontade de cuidar do filho no ambiente hospitalar. Os profissionais de saúde, com vistas a uma assistência integral, precisam compreender esta limitação, apoiar a mãe no atendimento de suas necessidades e ajudá-la superar esta condição.
