RESUMO
o aumento de coroa clínica estético representa um importante capítulo da plástica periodontal que, atualmente, tem sido bastante discutido. Tradicionalmente, o procedimento é realizado com a elevação de um retalho para a exposição da crista óssea e subsequente osteotomia/osteoplastia. Entretanto, em casos apropriados, é possível indicar um procedimento sem a elevação do retalho, possibilitando a realização de uma osteotomia via sulco gengival, com a utilização de micro-cinzéis apropriadas. Os benefícios da técnica incluem a finalização do procedimento sem a necessidade de suturas, a diminuição do desconforto pós-operatório e a otimização da reparação tecidual. Os resultados são previsíveis e esteticamente favoráveis, desde que se respeitem as indicações adequadas (biotipos fino e intermediário) e se obedeça criteriosamente o protocolo de execução.
ABSTRACT
Aesthetic crown lengthening is an important chapter of contemporary periodontology. Traditionally, this procedure is performed using flap elevation to fully expose the underlying bone and aliow subsequent osteotomv/osteoplastv However, in appropriately indicated cases, it is possible to meet that purpose in a flapless approach, i.e. without flap elevation, in which os- teotomy is performed through the gingival sulcus using proper microchisels. Among the benefits of this technique are the ability to finalize the procedure without the need for sutures, less postoperative discomfort and improved tissue repair. As long as the appropriate indications of the procedure are observed (thin and intermediate tissue biotypes) and the protocol is followed strictly, predictable and favorably aesthetic outcomes can be expected.
