RESUMO
Objetivos:
descrever um incomum caso de sífilis afetando a tonsila e revisar a literatura sobre o assunto.Descrição do caso um homem de 60 anos de idade, branco, apresentava tumefação no lado direito do pescoço e voz hiponasal quatro meses após ter sido submetido à cirurgia para remoção de adenoma na glândula submandibular esquerda. Ao exame físico apresentava linfadenopatia cervical, e uma tomografia computadorizada mostrou massa cervical. Foi realizada punção aspirativa por agulha fina para descartar linfoepitelioma, que não foi conclusiva. Como a tonsila direita era claramente patológica, foi realizada uma tonsilectomia. O diagnóstico de sífilis foi estabelecido com base na análise patológica combinada com resultados dos exames de sangue.Conclusões:
embora grande parte dos cancros sifilíticos apareça na região genital, eles podem ser vistos em outras partes do corpo. A falta de dor é um ponto chave da sífilis, juntamente com o relato do paciente da sensação de protuberância na garganta. Embora infrequente, a sífilis deve entrar no diagnóstico diferencial da hipertrofia tonsilar unilateral.ABSTRACT
Aims:
To report an unusual case of syphilis affecting the tonsil and to review the literature on the subject.Case description A 60 year-old Caucasian man presented a swelling of the right side of the neck, and hiponasal voice, four months following a surgery to remove an adenoma of the left submandibular gland. The physical examination showed cervical lymphadenopathy, and a computed tomography scan revealed a cervical mass. Fine-needle aspiration failed to rule out lymphoepithelioma. The right tonsil was clearly pathological and an extended right tonsillectomy was undertaken. A diagnosis of syphilis was established based on pathological analysis combined with blood tests results.Conclusions:
While many chancres appear on the genitals, they may be seen anywhere on the body. The hallmark of syphilis is the absence of pain and the patient?s feeling of a lump in the throat. Although infrequent, syphilis should enter in the differential diagnosis of unilateral tonsillar hypertrophy.
