RESUMEN
El delirium se caracteriza por fluctuación en el nivel de conciencia y deterioro de funciones cognoscitivas, con tres subtipos: hiperactivo, hipoactivo y mixto. Para el diagnóstico es suficiente la evaluación clínica. Existen instrumentos para tamizaje y seguimiento. La prevalencia en hospitalización en la CUB es de 8.3%. Los factores de riesgo producen un disbalance dopamina-acetilcolina. Para el manejo se deben modificar los factores etiológicos, proveer un ambiente familiar, garantizar la seguridad del paciente y de los demás; en cuanto al tratamiento farmacológico, el medicamento más usado es el haloperidol, que puede combinarse con lorazepam o midazolam; la olanzapina puede usarse como segunda opción al haloperidol. El delirum se asocia con mal pronóstico luego de una hospitalización, con alto riesgo de morir y de deterioro cognitivo y funcional. En cuanto al aspecto económico, está entre las complicaciones médicas que aumentan los costos en el manejo de los pacientes hospitalizados.
ABSTRACT
Deliriwn is charaeterized by flucntation in the leve! of consciousness and cognitive impairment, with tree subtypes: «hyperactive¼, «hypoactive¼ and «mixed¼. For the diagnosis, clinical evaluation is enough. There are instruments for screening and the follow up. The prevalence in the CUB in-patient service is 8.3%. Risk factors produce a misbalance between dopamine and acetylcholine. For treatment: etiological factors should be modified, the location should be as familiar as possible and the proteetion of the patient and othersshould be guaranteed. Regarding to the pharmacological treatment, the most widen used drug is haloperidol which can be combined with lorazepam or midazolam, olanzapine is the second option. Delirium is associated to poor prognosis after a hospitalization with a huge risk offunctional or cognitive damage, or even dying. From the economical point of view, this disorder is one of the medical complications that increases inpatients management costs.
