Um estudo de caso da implementação de um Centro de Atenção Psicossocial em álcool e drogas (CAPS-ad), no Rio de Janeiro, 2009-2010 / A case study of the implementation of a Psychosocial Care Center in alcohol and drugs (CAPS-ad), in Rio de Janeiro, 2009-2010

RESUMO

A questão que orientou a pesquisa que compõe esta tese foi a identificação e o estudo de fatores intervenientes na qualidade das ações de cuidado prestadas por um Centro de Atenção Psicossocial em álcool e drogas (CAPS-ad), na cidade do Rio de Janeiro. O estudo da experiência de implementação de um CAPS-ad visou destacar, nesta experiência, os itens pertinentes à qualidade das ações mencionadas. A qualidade do cuidado foi investigada na interface das diretrizes técnicas e políticas propostas para sua realização e o cotidiano deste tipo de serviço, talcomo experimentado por profissionais e usuários do mesmo. Os achados da pesquisa confirmam o que está sistematizado pela literatura nacional e internacional a respeito de ações públicas visando à redução de danos à saúde individual e coletiva, destinadas às pessoas que usam álcool e drogas de maneira prejudicial ou dependente. Tais ações são mais efetivas quando integradas a outras medidas de saúde pública, guiadas por princípios comuns. Iniciativas de redução de danos afinadas com princípios de saúde pública não se prendem a modelos, nem se esgotam em cuidados de saúde propriamente ditos. Abrangem diversas modalidades de ações práticas, com base em políticas públicas, devem estar em sintonia com a comunidade desde seu planejamento e devem ser executadas em parceria com esta.

ABSTRACT

The research question discussed in this dissertation was the identification and analysis of factors putatively fostering the provision of comprehensive care by a Psychosocial Center for People whoMisuse Drugs and Alcohol (known by its Portuguese acronym CAPS-ad), located in Rio de Janeiro. The case study describes the experience of implementation of a CAPS-ad, highlighting the items pertinent to the proposed model of comprehensive care and its translation into practice. The nature and quality of care were assessed in the context of technical guidelines and policies guiding its full implementation and its dialogue with theservice’s daily operations, as perceived by health professionals who work in this facility, as well as its clientele (patients and their families). The findings of the study document the adoption (not exempt of difficulties or obstacles) of policies and actions issued at the national level and documented in the international literature. Such policies aim to reduce drug-related harmsexperienced by individual and communities, and explicitly target people who use alcohol and drugs in a harmful way and/or are alcohol and/or drug dependent, as well as their families and peers. Such initiatives have been found to be effective when integrated to other public health measures, guided bycommon principles and aims. Such initiatives do not limit themselves to the context of health provision 4 nor remain in the realm of abstract models. Theyrather comprise concerted practical actions, in harmony with renewed public policies and the concrete needs of people who misuse drugs and alcohol, their families and peers, as well as their communities at large.
