Uma análise dos relatos sobre estados emocionais e motivacionais na evolução de um caso clínico / An analysis of accounts of emotional and motivational states in the development of a clinical case

RESUMO

O estudo teve como objetivo identificar a existência de relações entre referências a sentimentos, emoções ou estados motivacionais (SEM), na interação verbal entre terapeuta e cliente, e a evolução das queixas ou problemas relatados. Um caso clínico foi acompanhado pelo período de um ano, no qual foram analisadas as verbalizações de uma terapeuta analítico-comportamental e de uma cliente. Para investigar a evolução da terapia foram elaborados indicadores de queixa ou percepção de mudança. As referências aos SEM foram comparadas dentro e fora de episódios emocionais, definidos como sequências de diálogos entre terapeuta e cliente, nas quais houve, pelo menos uma referência a um SEM da cliente. Verificouse que os principais problemas da cliente eram o seu relacionamento conjugal e o comportamento pouco assertivo, mas a evolução desses problemas, assim como das demais queixas, não ocorreu sempre de forma regular ou positiva. Também não foi identificada uma relação mais consistente entre um determinado tipo de queixa e o relato de um SEM específico. Por outro lado, confirmouse que os terapeutas analítico-comportamentais não privilegiam os comportamentos públicos em detrimento dos privados, abordando os SEM de forma semelhante a qualquer outro comportamento. Finalmente, foram discutidos problemas metodológicos comuns à investigação de eventos privados no contexto terapêutico e sugeridas algumas mudanças para o aperfeiçoamento do método aqui empregado, que podem ser úteis a investigações futuras.

ABSTRACT

The study aimed to identify the existence of a relationship between references to feelings, emotions or motivational states (SEM) in verbal interaction between therapist and client and the evolution of the complaints or problems which the client reported. A clinical case was monitored over the course of a year, in which the verbalizations of a behavioranalytic therapist and a client were analyzed. In order to investigate the development of the therapy, indicators of complaint or perception of change were designed. The references to SEM were compared both inside and outside of the emotional episodes, defined as sequences of dialogue between therapist and client in which there was at least one reference to the client's SEM. It was found that the client's main problems were her marital relationship and her lack of assertiveness. However, the evolution of these problems, as well as the other complaints, did not always occur regularly or in a positive fashion. There was also no consistent relationship between a particular type of complaint and the report of a specific SEM. On the other hand, it was confirmed that the behavioranalytic therapists did not accord special importance to public behavior to the detriment of private behavior, and thus dealt with the SEM in a similar way to any other behavior. Finally, methodological problems, which were common to the investigation of private events in the therapeutic context, were discussed. Some changes were also suggested in order to improve the method used herein and which could be useful in future investigations.
