Tratamento conservador de trombose de prótese mecânica aórtica cardíaca / Conservative treatment of aortic thrombosis in a mechanical heart implant

RESUMO
Relata-se o caso de um paciente do sexo masculino, portador de válvula aórtica protética mecânica que, após interrupção da anticoagulação oral para realizar um procedimento cirúrgico não cardíaco, evoluiu com sinais clínicos de insuficiência cardíaca classe funcional II(NYHA), desaparecimento do click metálico aórtico e ecocardiografia transtorácica constatando a presença de trombo organizado na prótese aórtica com disfunção moderada. O tratamento adotado foi o não cirúrgico conservador com incremento no efeito cumarínico(INR>3,0). Paciente evoluiu com desaparecimento dos sinais de IC classe II, restabelecimento do click metálicoaórtico e ecocardiograma transesofágico evidenciando desaparecimento do trombo organizado.
ABSTRACT
This case study presents a male patient with a prosthetic mechanical aortic valve who, after discontinuation of oral anticoagulation in order to perform non-cardiac surgery, developed clinical signs of class II functional heart failure (NYHA) with the disappearance of the metallic click in the aorta. Transthoracic echocardiography indicated the presence of an organized thrombus in the aortic prosthesis with moderate dysfunction. The selected treatment was conservative and non-surgical, incrementing the coumarin effect (INR> 3.0). The patient progressed with the disappearance of the signs of class II HF, restoration of the aortic metallic click and transesophageal echocardiogram showing the disappearance of the organized thrombus.
