Hemangioma del nervio facial: Presentación de un caso y revisión de la literatura / Facial nerve hemangioma: Case presentation and literature review

RESUMEN

Objetivo: Los hemangiomas del nervio facial son tumores raros y en algunos casos sólo es posible diferenciarlos mediante histopatología. Este artículo presenta un caso clínico de hemangioma del facial en estadio tardío sin función facial residual y con hipoacusia conductiva, al cual se le resecó la lesión completamente sin recidiva tumoral. Diseño: reporte de caso y revisión de literatura. Resultados: Según la revisión de la literatura en estadios avanzados con degeneración total del facial, se recomienda una resección total con conservación de la audición, mediante un abordaje transmastoideo sin la reconstrucción primaria del nervio facial, puesto que no existen respuestas de amplificación de los potenciales de acción producidos por la fibra muscular en la electromiografía, presentando atrofia neuromuscular y nerviosa, lo cual limita el pronóstico en la recuperación de la función facial.Conclusión: La resección quirúrgica completa de hemangiomas del facial con reparación primaria del nervio es el tratamiento de elección. Pero es válida la conducta de resecar completamente el tumor, preservando la audición sin reparo primario del facial por atrofia de la placa neuromuscular y atrofia del facial, siendo inútil la reconstrucción de la porción resecada con el tumor.

ABSTRACT

Objective: The hemangiomas of the facial nerve are rare tumors and in some cases it is only possible to make the diagnosis through histopathology. This article shows a clinical case of a facial hemangioma in its late stage with no residual facial function and with conductive hearing loss. The tumor was fully resected without any tumor relapse. Design: Case report and literature review. Results: According to the information obtained from the literature review when the condition is in the advanced stages with the full degeneration of the facial nerve, it is recommended to perform a total resection of the tumor while preserving audition by a trans - mastoid approach without the primary reconstruction of the facial nerve. During the electromyography, the muscular fiber showed no responses from amplifying the action potentials. It exhibited a neural muscular and nerve atrophy, as well, which limits the prognosis during the recovery of the facial function. Conclusion: The complete surgical resection of hemangiomas of the facial nerve with the primary repair of the nerve is the treatment of choice. However, full resection of the tumor preserving audition without having to primarily repair the facial nerve is a valid approach, due to the atrophy of the neuromuscular plaque as well as to the atrophy of the facial nerve. It is useful to perform reconstruction of the portion that has been resected with the tumor.
