RESUMO
Objetivo:
Revisar a literatura científica disponível sobre os efeitos colaterais da terapia antirretroviral em crianças com HIV, focando a incidência e o tratamento da lipodistrofia. Fontes Pesquisadas Foi realizado um levantamento de artigos científicos nacionais e internacionais nas bases SciELO, Lilacs, Medline, Nature, PubMed e Bireme. Foram utilizadas as palavras-chave lipodistrofia, terapia anti-retroviral e crianças com HIV/AIDS, considerando-se somente artigos originais dos últimos 14 anos. Síntese dos Dados A AIDS continua sendo uma pandemia. Há no mundo 33 milhões de pessoas infectadas, com 2,5 milhões de crianças menores de 15 anos com HIV. Com o advento da terapia antirretroviral, teve-se um profundo impacto na história natural da infecção pelo HIV. As crianças infectadas convivem agora com uma doença crônica, ao invés de uma doença progressiva e fatal. Contudo, os efeitos colaterais da terapia causam alterações metabólicas, como a resistência à insulina, dislipidemia e lipodistrofia, elevando o risco de doença cardiovascular precoce.Conclusão:
Como os mecanismos da lipodistrofia não estão elucidados, ainda não se tem um consenso para a prevenção ou tratamento, sendo necessários mais estudos para contribuição no seu manejo. Mas é fato que uma alimentação saudável associada à atividade física e à adesão do paciente ao tratamento melhora a sobrevida e a qualidade de vida dessas crianças.ABSTRACT
Objective:
To review the available scientific literature about the side effects of antiretroviral therapy in children with HIV, focusing on the incidence and treatment of lipodystrophy. Data Source It was carried out a survey of scientific articles in national and international basis SciELO, Lilacs, Medline, Nature, PubMed, and Bireme. Were used the keywords lipodystrophy, antiretroviral therapy, and children with HIV/AIDS, considering only original articles of the past 14 years. DataSynthesis:
AIDS remains a pandemic. There are 33 million people infected in the world, with 2.5 million children under 15 years with HIV. With the advent of antiretroviral therapy, a profound impact on the natural history of HIV infection has arisen. The infected children now live with a chronic disease rather than a progressive and fatal disease. But the side effects of therapy cause metabolic changes such as insulin resistance, dyslipidemia and lipodystrophy, raising the risk of early cardiovascular disease.Conclusion:
As the mechanisms of lipodystrophy are not clear, there is still no consensus for the prevention or treatment, requiring more studies to contribute in their management. But the fact is that a healthy diet associated with physical activity and adherence to treatment improves survival and quality of life for these children.
