Costo hospitalario del ictus isquémico agudo / Hospital costs of acute ischemic ictus

RESUMEN

INTRODUCCIÓN: el costo por enfermedad es una prioridad investigativa del Sistema Nacional de Salud. OBJETIVO: determinar el comportamiento de los costos del ictus isquémico agudo en el Hospital Militar Central Dr. Carlos J. Finlay en Cuba. MÉTODOS: se realizó un estudio descriptivo retrospectivo mediante la revisión de las historias clínicas de 231 pacientes atendidos con ictus isquémico durante el periodo de un año. Las variables analizadas fueron: costo total de hospitalización, costo cama, gasto de medicamentos, costo de medios diagnósticos, estadía y complicaciones. RESULTADOS: el costo promedio por paciente con estadía hospitalaria menor de 9 días fue de 267,67 pesos cubanos y 647,39 cuando la estadía se prolongó más tiempo. La variable que más influyó en esta diferencia, fue el gasto de medicamentos. El costo de hospitalización por paciente en las salas de Terapia Intensiva e Intermedia fue aproximadamente el triple y el doble respectivamente, del costo de un paciente en la Sala de Neurología. CONCLUSIONES: la determinación realizada en el comportamiento de los costos, sirve de punto de referencia y constituye una herramienta adicional para valorar objetivamente el comportamiento de la enfermedad en la institución

ABSTRACT

INTRODUCTION: the disease burden is a researching priority of National Health System. OBJECTIVE: to determine the behavior of acute ischemia ictus costs in the Dr. Carlos J. Finlay Central Military Hospital in Cuba. METHODS: a retrospective and descriptive study was conducted through a review of medical records from 231 patients treated by ischemic ictus over a year. Variables analyzed were: hospitalization total cost, bed cost, drugs expense, diagnostic means cost, stay and complications. RESULTS: the mean burden by patient with hospital stay lower than 9 days was of 267.67 Cuban pesos and of 647.39 pesos when stay was larger. The variable that more influenced in this difference was the drugs expense. The hospitalization cost by patient in the intensive and intermediate care units was approximately the three times as much and the twice as much, respectively of the patient's burden admitted in the Neurology Service. CONCLUSIONS: the determination related to the costs behavior is the point of reference and is an additional tool to assess in an objective way the behavior of this disease in the institution
