Consideraciones sobre la cirugía endoscópica transluminal a través de un orificio natural / Considerations on transluminal endoscopic surgery through a natural orifice

RESUMEN

Desde finales del siglo pasado la cirugía ha sufrido un nuevo enfoque con el objetivo de ofrecer al paciente modalidades terapéuticas menos invasivas, lo que se ha dado en llamar cirugía mínimamente invasiva o de mínimo acceso. El 2007 se caracterizó por un incremento en el desarrollo de este tipo de proceder y apareció la cirugía endoscópica transluminal a través de un orificio natural, llamada cirugía sin huellas. Las alternativas de abordaje quirúrgico que hoy se proponen surgen ante la demanda de la sociedad de ofrecer al paciente métodos cada vez menos invasivos. Se requerirán estudios experimentales y clínicos para demostrar sus beneficios por encima de su alto costo, pero lo más importante será velar por el adecuado equilibrio entre la aceptación de hoy y los valores éticos y humanos que deben prevalecer en el camino hacia la cirugía sin huellas. En el presente trabajo se ofrecen algunas consideraciones al respecto, para lo cual se revisó lo publicado hasta el momento sobre el tema y se realizó un análisis de cómo los autores aprecian el fenómeno en la sociedad actual

ABSTRACT

From final of past century, surgery underwent a new approach to offer patient less-invasive therapeutic modalities, leading to the term minimally invasive surgery or of minimal access. The 2007 was characterized by an increase in the development of this type of procedure and appeared the transluminal endoscopic surgery through a natural orifice called surgery without traces. The alternatives of surgical approach nowadays proposed appear due to demand of society to offer patient methods less and less invasive. It will be necessary experimental and clinical studies to demonstrate its benefits over its high cost, but the more important will be to look after the appropriate equilibrium between the present acceptation and the ethical and human values prevailing in the route to surgery without traces. The objective of present paper is to offer some considerations in this respect, thus authors made a review of published information until nowadays on this subject and it was analyzed how the authors evaluate this phenomenon in current society
