Uso de midazolam y haloperidol en urgencias psiquiátricas / Midazolam and haloperidol use in psychiatric emergencies

RESUMEN

Introducción: Las emergencias comportamentales son la principal urgencia psiquiátrica, por el riesgo de agresión al entorno físico, a otras personas, al equipo de salud y al propio paciente. En Colombia es ampliamente utilizada la combinación de midazolam y haloperidol en el tratamiento de emergencias psiquiátricas y no se conoce evidencia médica que lo sustente. Objetivo: Encontrar evidencia que sustente este esquema terapéutico. Métodos: Búsqueda de estudios en las bases de datos PubMed, MedLine, HINARI, OVID, Lilacs, SciELO y PsycInfo, mediante la combinación de las siguientes palabras clave o términos MeSH emergency services, psychiatric, psychomotor agitation, midazolam y haloperidol. Como límites se seleccionaron los artículos desde el año 2000, de preferencia aquéllos de revisión sistemática, metaanálisis, ensayos clínicos controlados y revisiones de tema. Resultados: Se encontraron 146 artículos que relacionan el uso del midazolam y haloperidol en emergencias comportamentales, se leyeron los resúmenes y se seleccionaron los 11 artículos relacionados con el objetivo del estudio; se hizo una lectura crítica de éstos y no se encontró artículo alguno que sustente el uso del esquema midazolam y haloperidol en el tratamiento de la agitación psicomotora. Conclusiones: No se encontró ninguna evidencia en la literatura examinada que permita usar el esquema mencionado en emergencias psiquiátricas. Se hallaron artículos que muestran el uso de midazolam y haloperidol en diferentes esquemas terapéuticos. ABSTRACT

Introduction: Behavioral emergencies are the main psychiatric emergencies, due to the risk of aggressive behavior towards other people, healthcare professionals, the physical environment and the patient himself. In Colombia, the combination of midazolam and haloperidol is widely used in the management of this type of emergencies. Evidence supporting its use is not available. Objective: The aim of this study is to find evidence that supports the application of this therapeutic combination. Methods: A search was made in databases such as PubMed, MedLine, HINARI, OVID, Lilacs, SciELO, and PsycInfo. The combinations of keywords used in this search in MeSH Database were Emergency services, psychiatric, psychomotor agitation, midazolam, and haloperidol. Applied limits were published articles from 2000, systematic reviews, meta-analysis, controlled clinical trials and reviews. Results: The search rendered 146 articles in relation to the use of haloperidol and midazolam in behavioral emergencies. The abstract of each article was carefully read and 11 of them were selected. These articles were later analyzed but no article was found that supports the use of midazolam in combination with haloperidol in the treatment of psychomotor agitation. Conclusions: There is not available evidence in the reviewed literature that allows the use of the aforementioned combination in psychiatric emergencies. There were only articles that compare midazolam and haloperidol between them or with other therapeutic options.
