RESUMEN
Las diferentes pruebas procalcitonina semicuantitativa, hemocultivos y hemograma fueron realizadas en el laboratorio del hospital. Los datos fueron recolectados, por los autores, en un formulario estructurado y analizados con el software SPSS. De los casos estudiados 44 neonatos presentaron el 58.7% fueron de sexo masculino y 31(41.3%) de sexo femenino. De los 15 neonatos que resultaron positivos al hemocultivo 10 fueron hombres (66.7%) y 5 mujeres (33.3%). La diferencia de proporciones dio un X2 de 0.495, con una p=0.482. En lo que respecta a los factores de riesgo de sepsis neonatal relacionados con la madre y con el recién nacido no se encontraron diferencias estadísticamente significativas. Con relación a otros criterios para diagnóstico sepsis neonatal el recuento plaquetario proporcionó un Chi cuadrado de 10.0, con una p=0.005 (Test por continuidad de Yates); RP 3.9 (IC 95% 1.7-9.2). El recuento leucocitario proporcionó un valor p=0.001 (Test exacto de Fisher), RP 6.5 (IC 95% 3.7-11.1). El valor del PCR proporcionó una p=0.024 (Test exacto de Fisher), RP 4.4 (IC 95% 2.1-9.5). Del total de la muestra 15 neonatos que tuvieron hemocultivo positivo, que representó el 20%. En cuanto a los resultados proporcionados por la prueba de la procalcitonina, se encontró una sensibilidad del 93.3% (IC 95% 89.92-96.74), una especificidad del 38.3% (IC 95% 37.43-39.24), un valor predictivo positivo de 27.5% (IC 95% 26.40-28.51) y un valor predictivo negativo del 95.8% (IC 95% 93.70-97.97).Con el punto de corte de 0.5ng/dl utilizado para el análisis en nuestro estudio; la prueba de PCT tiene una alta sensibilidad (93.3%), sin embargo la especificidad es baja (38.3%), lo cual indica que hay un alto porcentaje de falsos positivos.
ABSTRACT
I realized a study for evaluation of capacity of discrimination of diagnoses test, in a population of 75 New born with weight less than 2000 grams, entered the unit of neonatologica of the Hospital Vicente Corral Moscoso. The training group was conformed by neo-born of age between 6 and 24 hours of life. The different tests semi-quantitative procalcitonina, hemo-crops and hemograma were realized in the laboratory of the hospital. The data were collected, by the authors, in a structured form and analyzed with software SPSS. Of the studied cases 44 neo-born ones that it represented the 58,7% were of masculine sex and 31(41,3%), of feminine sex. Of the 15 neo-born ones that was positive to the hemo-crops 10 were men (66,7%) and 5 women, the 33,3%. The difference of proportions gave a x2 of 0,495, with one p=0.482. With regard to the risk factors of neonatal sepsis related to the mother and to new born was not significant statistically difference found. With regard to other criteria for neonatal diagnoses sepsis the plaquetario count provided a squared Chi of 10,0, with one p=0.005 (Test by continuity of Yates); RP 3,9 (IC 95% 1.7-9.2). The leucocytary count provided a value p=0.001 (exact Test of Fisher), RP 6,5 (IC 95% 3.7-11.1). The value of the PCR provided one p=0.024 (exact Test of Fisher), RP 4,4 (IC 95% 2.1-9.5). Of the total of the sample 15 neo-born that had positive hemo-crop that represented 20%. As far as the results provided by the test of the procalcitonina, it was found a sensitivity of the 93,3% (IC 95% 89.92-96.74), a specificity of the 38,3% (IC 95% 37.43-39.24), a positive predictive value of (27,5% IC 95% 26.40-28.51) and a negative predictive value of the 95,8% (IC 95% 93.70-97.97).With the point of it cuts of 0.5ng/dl used for the analysis in our study; the test of PCT has a high sensitivity (93,3%), nevertheless the specificity is low (38,3%), which indicates that there is a high percentage of false positives.
