Construção de ficha de notificação de acidentes e violências e reprodutibilidade interobservadores / The development of a recording accidents and violence card and interobservers reproducibility

RESUMO

Objetivo - Construir uma ficha de notificação de agravos em vítimas de acidentes e violências, para serviços de urgência e emergência e avaliar a reprodutibilidade interobservadores. Métodos - Sua primeira versão foi construída a partir de pesquisa bibliográfica. Em seguida, foi avaliada por especialistas e testada em estudo piloto. Posteriormente foi preenchida no serviço. Foram entrevistadas e reentrevistadas 73 vítimas. Mensurou-se a reprodutibilidade pelo percentual de concordâncias e pelo kappa bruto e ajustado. Resultados - A ficha de notificação foi facilmente preenchida (média 6 minutos e 54 segundos; mediana - 5 minutos). A proporção de concordâncias ficha/reentrevista variou entre 40 e 100%. Utilizando-se o kappa, indicadores como agressões e alta obtiveram piores desempenhos (0,37 e 0,34). Os demais mantiveram concordância entre 0,68 e 1,00. Com seu ajustamento para viéses e prevalência obtiveram-se melhores resultados (entre 0,73e 1,00). Para duas questões foi detectada necessidade de reestruturação. Conclusões - O instrumento foi construído e obteve boa reprodutibilidade interobservadores na maioria das questões.

ABSTRACT

Objective - To develop a recording card presenting the damages in violence and accident victims in urgency and emergence services and to evaluate the interobservers reproducibility in its use. Methods - The first version of the recording card was built through a bibliographic research about the theme. Afterwards, it was evaluated by specialists and tested in a sample study. Lately it was filled in by patients who suffered injuries by external causes. Seventy-three victims were interviewed and re-interviewed. It was performed the reproducibility measurement through the agreement percentage and the kappa index adjusted and gross. Results - The recording card was easily fulfilled (average= 6 minutes 54 seconds; median - 5:00 minutes). The crude proportion of agreement record/re-interview ranged between 40 and 100%. According to the Kappa Agreement Index, indicators as aggressions and discharge obtained the worst results (0,37 and 0,34). The others kept the agreement index between 0,68 and 1,00. After adjusting kappa to bias and to prevalence, results of better quality were obtained (between 0,73 and 1,00). Two questions needed to be restructured. Conclusions - The instrument was built and in the most of the questions interobserves reproducibility was obtained.
