O uso da contratransferência no tratamento psicoterápico de pacientes graves: a propósito de um caso clínico / The use of countertransference in the psychotherapic treatment of severe ill patients: a clinical case

RESUMO

É consenso na literatura que houve uma notável mudança no perfil dos pacientes que buscam tratamento psicoterápico nas últimas décadas. Tal mudança torna mandatório o questionamento sobre a necessidade de adequações técnicas que atendam às vicissitudes inerentes a esses pacientes. Ilustrado por um caso clínico, este trabalho aborda alguns aspectos técnicos preconizados para a compreenção e para manejo psicoterápico dos pacientes graves, salientando o uso da contratransferência como ferramenta relevante. A partir dos conteúdos estudados, conclui serem dois os aspectos de consenso entre os autores o primeiro refere-se à importância de que o foco de atenção no manejo desses pacientes seja a não-rigidez, a flexibilização da técnica, a maior disponibilidade emocional do terapeuta e seu autoconhecimento, para que utilize ao máximo seus sentimentos contratransferenciais na sua compreensão e abordagem. O segundo aspecto, sobre o qual há concordância, relacion-se à necessidade de que cada novo instrumento utilizado seja investigado profundamente e com seriedade para verificação de sua validade.

ABSTRACT

It is consensus in the literature that a remarkable change has occured in the profile of patients that look for psychotherapic treatment in the last decades. Such change makes it mandatory to question the necessity of having techniques adjustments in order to attend to the patients' vicissitudes. Illustrated with a clinical case, this paper approaches some technical aspects recommended for the comprehension and management of the severely ill patients, emphasizing the use of countertransference as a relevant instrument. From the revised literature, the author concludes that there are two aspects of consensus among the authors; the first one refers to the importance that the focus of attention in the management of these patients must be on the no rigidity, on the flexibility of the technique, on the therapist's larger emotional availability and self-knowledge, allowing him/her to use their countertransferencial feelings to the maximum. The second aspect about which the authors agree with refers to the need to deeply and seriously investigate any new instrument used, in order to check its validation.
