RESUMEN
Objetivo:
La escasez de psiquiatras y el incremento de la demanda de atención psiquiátrica motivó a investigar la posibilidad que trabajadores de salud no especialistas (TSNE), detecten casos psiquiátricos por medio del Cuestionario de Autoreporte (CAR), previa evaluación del punto de corte e investigación de la confiablidad, validez y posibilidades futuras del instrumento.Material:
Se aplicó el CAR a 497 trabajadores de una institución militarizada y a 2 submuestras de 64 pacientes psiquiátricos y 157 sujetos supuestamente sanos. Las evaluaciones fueron realizadas por 8 TSNE capacitados y por psiquiatras.Resultados:
La confiabilidad del cuestionario en pacientes psiquiátricos entre los TSNE y el psiquiatra investigador, fue de 88% (a=0.88, K=0.76, Z=28.25, p<0.001); la validez concurrente con los psiquiatras tratantes fue de 83% (a=0.83, k=0.65, z=20.96, p<0.001), menor con el psiquiatra investigador, en sujetos supuestamente sanos (a=0.62, K=0.62, Z=2.84, p<0.001). En la subescala de neurosis, la correlación entre la frecuencia de respuestas afirmativas o negativas y la catalogación de caso y no caso fue altamente significativa en todas las preguntas. Las proporciones de los casos en relación a los subtotales, cuando el síntoma está presente, es superior al 50% en todas las preguntas excepto en las preguntas 7 y 8.Conclusiones:
La confiabilidad y validez concurrente y de constructo teórico, resultaron apropiadas para el universo del cual procede la muestra, recomendándose usar el CAR en el seguimiento de pacientes psiquiátricos y estudio de poblaciones en riego (desastres) o que no dispongan de especialistas.ABSTRACT
Objectives:
To investigate if non-specialized health workers (NSHW) could discover psychiatric cases using Self Reporting Questionaire (SRQ). The cut point, reliability, validity and future possibilities of this instrument were investigates.Methods:
The SRQ was applied to 497 workers of an Army Institute. Subgroups of 64 psychiatric patients and 157 supposed healthy subjects were also evaluated by 8 NSHW and psychiatrics.Results:
The reliability between the NSHW and the Psychiatric investigator (PI) was 88% (a=0.88, K=0.76, Z=28.25, p<0.001) in the psychiatric patients group. The validity with the attending Psychiatric (AP) was 83% (a=0.83, k=0.65, z=20.96, p<0.001), the results in the PI with supposed healthy subjects was lower than the AP (a=0.62, K=0.62, Z=2.84, p<0.001). The correlation between the rate of affirmative or negative answers with the case and no case identification was highly significative for the neurosis sub-scale. When symptoms are present, the cases proportion related to the subtotal scores is higher than 50 percent for all items, excepting item 7 and 8.Conclusions:
The reliability and validity of this tool was demonstrated. The application of the SRQ is appropriate in the follow-up of psychiatric patients, as well as in research studies of people in risk (disasters) and in people without specialized attention.
