RESUMEN
El uso de anticonceptivos, especialmente de los que contienen estrógenos por parte de mujeres con lupus eritematoso sistémico (LES) conlleva riesgos como exacerbaciones de la enfermedad, trombosis u otros efectos adversos. Sin embargo, de acuerdo con la evidencia disponible, muchas mujeres con LES, especialmente aquellas con enfermedad estable, no tienen un alto riesgo de exacerbaciones de la enfermedad y por lo tanto pueden ser buenas candidatas para diversos métodos anticonceptivos, incluyendo los hormonales. Por el contrario, las mujeres con anticuerpos antifosfolípidos positivos presentan un alto riesgo de trombosis arterial y venosa y en consecuencia se debe evitar en ellas el uso de anticonceptivos hormonales combinados.
ABSTRACT
The use of contraceptives, particularly of those containing estrogens by women with systemic lupus erythematosus (SLE), has been thought to carry risks such as disease exacerbation, thrombosis and other adverse effects. However, the available evidence suggests that many women with SLE, particularly those with stable disease, are not at increased risk of disease flare while taking oral contraceptives and therefore can be good candidates for most contraceptive methods, including the hormonal ones. Contrariwise, women with positive antiphospholipid antibodies are at increased risk of arterial and venous thrombosis and therefore the use of combined hormonal contraceptive methods should be avoided in them.
