Fundamentos A pressão central reflete com mais precisão as condições de sobrecarga ao ventrículo esquerdo do que as aferições sobre a artéria braquial, nos pacientes hipertensos. Objetivo:
Comparar a pressão de pulso (PP) obtida na raiz da aorta com a aquela obtida indiretamente na artéria braquial, em hipertensos controlados (CTL) e não controlados (NCTL).Métodos:
Trata-se de estudo transversal com pacientes admitidos eletivamente, no período de 2009 a 2012, para realização de estudo cinecoronariográfico. Os pacientes foram estratificados em dois grupos CTL quando os valores da pressão arterial (PA) <140×90 mmHg (n=110); e NCTL quando os valores da PA ≥140×90 mmHg (n=120) em uso de medicamentos hipotensores por via oral.Resultados:
Tanto no grupo CTL quanto no NCTL as variáveis da pressão arterial sistólica e a PP foram significativos a partir dos 50 anos de idade. No presente estudo 52,0±6,0 % pacientes estavam no grupo NCLT, sugerindo que um grande número de indivíduos ainda se mantenha exposta a doenças graves como o acidente vascular encefálico e a doença arterial coronariana.Conclusão:
Com o envelhecimento, notadamente a partir dos 50 anos de idade, a PS e PP aferidas diretamente na raiz da aorta são maiores do que as pressões respectivas aferidas indiretamente na artéria braquial. O diagnóstico e o controle da HAS, se baseados exclusivamente nos níveis obtidos pela PAb podem ser postergados em pelo menos uma década.ABSTRACT

To compare pulse pressure (PP) at the aortic root with that obtained indirectly in the brachial artery in controlled (CTL) and uncontrolled (NCTL) hypertensive patients.Methods:
This is a cross-sectional study of patients admitted electively for coronary angiography between 2009 and 2012, divided into two groups CTL with blood pressure (BP) values of <140×90 mmHg (n=110) and NCTL with BP values of ≥140×90 mmHg, taking oral antihypertensive medications (n=120).Results:
For both the CTL and NCTL groups, systolic blood pressure and PP variables were significant from 50 years of age onwards. In this study, 52.0±6.0% of the patients were in the NCLT group, suggesting that large numbers of people are still exposed to serious diseases such as stroke, and coronary artery disease.Conclusion:
With aging, particularly from 50 years of age onwards, the SBP and PP measured directly at the root of the aorta are higher than the respective pressures measured indirectly in the brachial artery. If based solely on the levels obtained in the brachial artery, diagnosis and control of hypertension may be delayed by at least a decade.
